Special Report, Channel #12 Containment Hydrogen Monitoring System Inoperable Due to Degraded Solenoid Valve Gentlemen:

In accordance with Action Statement 4.a of Nine Mile Point Unit 1 Technical Specification Table 3.6.11-2, Accident Monitoring Instrumentation Action Statements, Nine Mile Point Nuclear Station, LLC is submitting the following Special Report concerning the inoperability of the Channel #12 Containment Hydrogen Monitoring System.

Description of Event On October 16, 2002, during preparation for the calibration of the Containment Hydrogen Monitoring System, a chattering noise was noted coming from the Channel #12 Containment Hydrogen Monitoring System cabinet. Further investigation revealed that the source of the noise was solenoid operated valve SOV-201.2-1248. At approximately 1434 hours0.0166 days <br />0.398 hours <br />0.00237 weeks <br />5.45637e-4 months <br />, operators shut down the Channel #12 Containment Hydrogen Monitoring System and the chattering ceased

Cause of Event Troubleshooting concluded that SOV-201.2-1248 was degraded. An inspection of the solenoid internals determined that the degradation was the result of normal wear.

1. The solenoid operated valve was replaced.
2. On October 25, 2002, at 2000 hours0.0231 days <br />0.556 hours <br />0.00331 weeks <br />7.61e-4 months <br />, Channel #12 Containment Hydrogen Monitoring System was returned to service.
3. The solenoid valve failure has been entered into the site's corrective action program to assess the need for additional follow-up actions.
