Unit 1 Docket No. STN 50-528 Special Report 1-SR-2002-003 Attached please find Special Report 1-SR-2002-003 prepared and submitted pursuant to the PVNGS Offsite Dose Calculation Manual requirements. This report discusses the inoperability of the fuel building ventilation system high range radioactive gaseous effluent monitor for more than seventy-two (72) hours.

Palo Verde Nuclear Generating Station Unit I Special Report No. I-SR-2002-003 Fuel Building Ventilation Radioactive Gaseous Effluent High Range Monitoring Instrumentation Inoperable Docket No. STN 50-528 Initial Conditions:

At approximately 2055 MST on October 13, 2002, Palo Verde Unit 1 was in a refueling outage, with the core off-loaded to the spent fuel pool, when the fuel building Radioactive Gaseous Effluent High Range Monitor (RU-146) was declared inoperable due to its 120-vac power supply (PNB-D26) being removed from service for planned maintenance.

This Special Report is being submitted pursuant to the ODCM requirement 2.1, ACTION 42(b) to prepare and submit a Special Report to the Commission within 30 days if the monitor is not restored within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />. The 72 hour8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> period for returning the monitor to an operable status expired at approximately 2055 MST on October 16, 2002 with the monitor not operable.

Actions Taken:

The Preplanned Alternate Sampling Program for RU-146 to monitor the fuel building ventilation was completed pursuant to the ODCM requirement 2.1, ACTION 42(a) on October 16, 2002 at approximately 1730 MST. On October 17, 2002 at approximately 0321 MST, the 120-vac power supply was returned to service and the monitor was declared OPERABLE.

Cause of the Inoperability:

There was no malfunction of the monitor associated with the inoperability. The cause for exceeding the 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> allowed by ODCM 2.1, Action 42(b) was the lack of consideration during the cross discipline reviews when planning the coordination of maintenance activities to support the action statement. Power had been restored to the bus supplying RU-146 in 67.1 hours1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />; however, the supply breaker for RU-146 had been removed for maintenance and was not returned to service till after the maximum allowed time for the action.

Plans and Schedule for Restoring the Channels to OPERABLE Status:

On October 17, 2002 at approximately 0321 MST, the monitor power supply was returned to service and the monitor was declared OPERABLE. The total elapsed time of the inoperability was 78 hours9.027778e-4 days <br />0.0217 hours <br />1.289683e-4 weeks <br />2.9679e-5 months <br /> and 26 minutes.
