SUMMARY
OF THE JULY 25, 2002, D. C. COOK PUBLIC MEETING
Dear Mr. Bakken:
On July 25, 2002, the Nuclear Regulatory Commission (NRC) held a public meeting at the NRC Region III office in Lisle, Illinois. The meeting was a Regulatory Conference to discuss a preliminary Yellow finding involving the essential service water debris intrusion event that occurred at the D.C. Cook Nuclear Power Plant on August 29, 2001. A Yellow finding is described as having substantial safety significance. Formal presentations were made by the Indiana Michigan Power Company (licensee) staff. Enclosure 1 is a list of attendees for the meetings, and a copy of the slides used by the licensee is provided as Enclosure 2.
Your Site Vice President opened the conference and remarked that the maintenance practices on the essential service water (ESW) pump discharge strainer basket that led to the event were unacceptable. However, your staffs internal evaluation concluded that the debris intrusion event was less significant than the preliminary Yellow level initially determined by the NRC.
Your Regulatory Affairs Manager provided an overview of the event, including the sequence of events, root and contributing causes, and corrective actions. Your System Engineering and Event Recovery Managers discussed a revised internal risk significance determination assessment of the finding. As a result of new information and considerations, your staff revised the overall probability scale used to characterize inputs to the assessment and the probability that sufficient debris was available in the forebay for ingestion into the essential service water system. Your staff also clarified their assumption that the operator recovery actions should be considered independent for each of the two units. Based upon these changes, your staff concluded that the core damage frequencies for both Units 1 and 2 and the change in large early release frequency for Unit 2 were below the greater-than-Green threshold. The change in large early release frequency for Unit 1 was determined to be marginally above the Green-to-White threshold. However, your staff stated that conservatisms regarding charging system unit cross tie capabilities and availability of 69 kV offsite power were not credited in your latest significance determination assessment.
A. Bakken Your Nuclear Technical Services Director summarized the presentation stating that the overall risk significance of the essential service water debris intrusion event was Green. Closing remarks were provided by you and your Site Vice President, when you reiterated that the significance of the debris intrusion event was not Yellow as preliminarily determined by NRC.
