Dear Mr. Randolph:
Enclosed is a copy of the Federal Emergency Management Agency's (FEMA) exercise evaluation report of the October 3, 2001, emergency preparedness exercise at the Callaway Nuclear Power Plant.
The report indicates that FEMA observed no deficiencies and two areas requiring corrective actions (ARCAs) during the exercise. The ARCAs were successfully redemonstrated and closed.
The purpose of this letter is to transmit to you the results of the FEMA evaluation of the emergency exercise. No response to the NRC is required.

I. EXECUTIVE
SUMMARY
On October 3,2001, an exercise was conducted in the plume exposure pathway emergency planning zone (EPZ) around the Callaway Nuclear Power Plant by the Federal Emergency Management Agency (FEMA), Region VIT. In addition, on August 13-14 and October 2,2001, four out-of-sequence drills were conducted The purpose of the exercise and drills was to assess the level of State and local preparedness in responding to a radiological emergency. The exercise and drills were held in accordance with FEMA's policies and guidance concerning the exercise of State and local radiological emergency response plans and procedures.
The previous exercise at this site was conducted on September 14, 1999. The qualifying emergency preparedness exercise for final plan approval was conducted on March 21, 1987.
FEMA wishes to acknowledge the efforts of the many individuals who participated in this exercise. In the State of Missouri, the counties of Callaway, Gasconade, Montgomery, and Osage participated along with various organizations of the State government. The efforts of the utility should also be commended for their work on the scenario development and exercise preparation.
Protecting the public health and safety is the full-time job of some of the exercise participants and an additional assigned responsibility for others. Still others have willingly sought this responsibility by volunteering to provide vital emergency services to their communities.
Cooperation and teamwork of all the participants was evident during this exercise. However, in the interest of brevity, only inadequacies are discussed in this report.
This report contains the evaluation of the biennial exercise.
The State and local organizations, except where noted in this report, demonstrated knowledge of their emergency response plans and procedures and adequately implemented them. There were no Deficiencies and two Areas Requiring Corrective Action (ARCAs) identified as a result of this exercise. After additional training was provided, both ARCAs were successfully re-demonstrated and are closed.
There was one ARCA identified during the previous exercise conducted September 14, 1999. It was corrected by a plan change.
The final protective action decision (PAD) during the emergency phase was evacuation of a 5-mile radius and the south, southeast, southwest, and northeast portion of the IO-mile EPZ out to ten miles. The evacuated subzones were C1, C2, C3, C4, C5, C6, C7, C1I, G1, M2, and 01.
The evacuated area included the towns of Reform, Readsville, Steedman, Mokane, Portland, Bluffton, and Morrison. Approximately 5,600 residents and transients were affected by the evacuation.

II. INTRODUCTION On December 7, 1979, the President directed FEMA to assume lead responsibility for all offsite nuclear planning and response. FEMA's activities are conducted pursuant to 44 Code of Federal Regulations (CFR) Parts 350, 351, and 352. These regulations are a key element in the Radiological Emergency Preparedness (REP) Program that was established following the Three Mile Island Nuclear Station accident in March 1979.
FEMA Rule 44 CFR 350 establishes the policies and procedures for FEMA's initial and continued approval of State and local governments' radiological emergency planning and preparedness for commercial nuclear power plants. This approval is contingent, in part, on State and local governments' participation in joint exercises with licensees.
FEMA's responsibilities in radiological emergency planning for fixed nuclear facilities include the following:
Taking the lead in offsite emergency planning and in the review and evaluation of radiological emergency response plans (RERP) and procedures developed by state and local governments.
Determining whether such plans and procedures can be implemented on the basis of evaluation of exercises ofthe plans and procedures conducted by state and local governments.
Responding to requests by the U.S. Nuclear Regulatory Commission (NRC) pursuant to the Memorandum of Understanding between the NRC and FEMA (Federal Register, Vol. 58, No. 176, September 14, 1993).
Coordinating the activities ofthe following federal agencies with responsibilities in the radiological emergency planning process:

- U.S. Department ofthe Interior Representatives of these agencies serve as members of the FEMA Region vn Regional Assistance Committee (RAC), which is chaired by FEMA.

Formal submission of the RERPs for the Callaway Nuclear Power Plant to the RAC, by the stfl,te of Missouri and involved local jurisdictions, was followed by a critique and evaluation ofthose plans. Formal approval ofthese plans was granted by FEMA on July 9, 1985. The Alert and Notification System was formally granted approval by FEMA on May 18, 1987.
A REP exercise was evaluated on October 3,2001, by FEMA Region vn to assess the capabilities of State and local offsite emergency preparedness organizations in implementing their RERPs and procedures to protect the public during a radiological emergency involving the Callaway Nuclear Power Plant. The purpose of this exercise report is to present the exercise results and findings on the performance of the offsite response organizations (OROs) during a simulated radiological emergency.
The findings presented in this report are based on the evaluations of the Federal evaluator team, with final determinations made by the FEMA Region vn RAC Chairperson and approved by the Regional Director.

Section ill of this report, entitled "Exercise Overview," presents basic information and data relevant to the exercise. This section of the report contains a description of the plume EPZ, a listing of all participating jurisdictions and functional entities evaluated, and a tabular presentation of the time of actual occurrence of key exercise events and activities. Federal participants were not evaluated.
Section N ofthis report, "Exercise Evaluation and Results," presents basic information on the demonstration of applicable exercise criteria at each jurisdiction or functional entity evaluated in a jurisdiction-based, issues only format. This section also contains: (1) descriptions of all Deficiencies and ARCAs assessed during this exercise, recommended corrective actions, and the State and local governments' response and (2) descriptions of ARCAs assessed during previous exercises and the ORO's schedule of corrective actions for each identified exercise issue.

III. EXERCISE OVERVIEW Contained in this section are data and basic information relevant to the October 3,2001, exercise and out-of-sequence drills to test the offsite emergency response capabilities in the area sur-rounding the Callaway Nuclear Power Plant. This section ofthe exercise report includes a description of the plume EPZ, a listing of all participating jurisdictions and functional entities evaluated, and a tabular presentation ofthe time of the actual occurrence of key exercise events and activities.
A. Plume Emergency Planning Zone (EPZ) Information The Callaway Nuclear Power Plant is located in Callaway County, Missouri, ten miles southeast of Fulton, 25 miles northeast of Jefferson City, and 80 miles west of St. Louis. The site is four and a half miles north of the Missouri River on a plateau 300 feet above river level. It is owned and operated by the Ameren Union Electric Company, St. Louis, Missouri.
The 10-mile EPZ contains a total population of approximately 16,635 within the four risk counties of Callaway, Gasconade, Montgomery, and Osage. The land use within the EPZ is predominantly rural/agricultural. Within the 5-mile radius ofthe plant, the topography consists of 60% forest, 20% farm/crops, and 20% pasture. Various forms of transportation serve the area.
Interstate Highway 70 passes within 12 miles to the north ofthe site, U.S. Highway 54 passes within 13 miles to the west, and Amtrak passes approximately five miles to the south along the Missouri River.
The EPZ is divided into 15 subareas within the four counties listed above. In addition to Fulton, eleven towns are within the EPZ; Reform, Readsville, Steedman, Mokane, Portland, Deer, Chamois, Calwood, Rhineland, Bluffton, and Morrison.

IV. EXERCISE EVALUATION AND RESULTS Contained in this section are the results and findings of evaluation of all jurisdiction and functional entities that participated in the August and October 2001 out-of-sequence drills and the October 3, 2001, exercise to test the offsite emergency response capabilities of State and local governments in the 10-mile EPZ surrounding the Callaway Nuclear Power Plant.
Each jurisdiction and functional entity was evaluated on the basis of its demonstration of exercise criteria delineated in the FEMA Exercise Evaluation Areas and Criteria, as published in the Federal Register, September 12,2001. Detailed information on the exercise criteria and the extent-of-play agreement for this exercise is found in Appendix 3 ofthis report.
A. Summary Results of Exercise Evaluation The matrix presented in Table 2, on the following page, presents the status of all exercise criteria from the FEMA Exercise Evaluation Areas and Criteria which were scheduled for demonstration during this exercise, at all participating jurisdictions and functional entities. A Deficiency is defIned by FEMA as flan observed or identified inadequacy of organizational perfonnance in an exercise that could cause a finding that offsite emergency preparedness is not adequate to provide reasonable assurance that appropriate protective measures can be taken in the event of a radiological emergency to protect the health and safety of the public living in the vicinity of a nuclear power plant. fI" 12
An Area Requiring Corrective Action (AReA) is defined by FEMA as "an observed or identified inadequacy of organizational performance in an exercise that is not considered, by itself, to adversely impact public health and safety."
FEMA has developed a standardized system for numbering exercise issues (Deficiencies and ARCAs). This system is used to achieve consistency in numbering exercise issues between FEMA Regions and site-specific exercise reports within each Region. It is also used to expedite tracking of exercise issues on a nationwide basis.
The identifying number for Deficiencies and ARCAs includes the following elements, with each element separated by a hyphen (-).
- Plant Site Identifier - A three-digit number corresponding to the Utility Billable Plant Site Codes.
- Exercise Year - The four digits of the year the exercise was conducted.
- Criterion Number - A three-digit number corresponding to the criteria numbers in the FEMA Exercise Evaluation Areas.
- Issue Classification Identifier - (D = Deficiency, A = ARCA). Only Deficiencies and ARCAs are included in exercise reports.

The State Emergency Operations Center (SEOC) is located in the Ike Skelton Training Center in Jefferson City, Missouri. The Emergency Alert System (EAS) radio station, KTXY, is located in Columbia, Missouri. The State EOC is an outstanding facility. The Operations Officer provided excellent direction and control. The staff of KTXY was very knowledgeable of their role and responsibilities.

Appropriate contamination control measures were not adequately demonstrated. A member of the ambulance crew failed to replace or simulate replacing outer gloves, in accordance with their procedures, following removal of a possible contaminated outer pair of gloves. Later, one of the remaining gloves developed tears that exposed the skin. This could have resulted in a cross contamination situation. In addition, the crew was not aware of the location that they and the ambulance would be monitored and decontaminated if required.

Appendix 3 - Exercise Criteria and Extent of Play Agreement This appendix lists the exercise criteria which were scheduled for demonstration during the Callaway Nuclear Power Plant exercise on October 3,2001, and the out-of-sequence drills on August 13-14 and October 2,2001. The Missouri State Emergency Management Agency submitted offsite criteria for this exercise during the Extent of Play Meeting on March 14,2001, which were subsequently approved by FEMA Region VII in the Extent of Play Letter on June 27, 200l.
The exercise criteria, contained in the Federal Register, September 12,2001, represent a functional translation ofthe planning standards and evaluation criteria ofNUREG-0654IFEMA-REP-I, Rev. 1, "Criteria for the Preparation and Evaluation of Radiological Emergency Response Plans and Preparedness in Support of Nuclear Power Plants," November 1980.
Because the exercise criteria are intended for use at all nuclear power plant sites and because of variations among offsite plans and procedures, an extent-of-play agreement dated June 27,2001, was prepared by FEMA Region VII and provided to the State of Missouri for further clarification of expected demonstration ofthe criteria.
Listed below are the specific REP criteria scheduled for demonstration during this exercise and the extent of play agreement, if applicable.
The following comments are provided for clarification of exercise requirements regarding the exercise criteria indicated as we discussed in the extent of play meeting held on March 14, 200l.
These comments are in addition to the generic extent of play for each exercise criteria. In addition, the dates and times for the out of sequence demonstrations agreed upon since the extent of play meeting are included for your information. The press releases to announce the public meeting will be provided to the media by our Public Information Officer and a copy will be provided to your office.
Criterion lal - Mobilization: All telephone calls to mobilize personnel or place them on standby must be actually made. Prepositioning of staff is not authorized, except by written approval from this office prior to the exercise. Your request must include which staff positions will be prepositioned and which facility(ies) will be affected.
The demonstrations at the Fulton Public School District #58, Osage R-I School District, Hermann Reception and Congregate Care Center, Callaway Community Hospital, and. Callaway County Ambulance District will occur out of sequence and all personnel at these facilities may be prepositioned. The evaluators for the Radiological Field Monitoring Teams will meet the teams at the Missouri Department ofHealthlEnvironmental Public Health Office on October 3,2001, at 0800 hours0.00926 days <br />0.222 hours <br />0.00132 weeks <br />3.044e-4 months <br />. The issuance of radiological instrumentation and operability checks, where applicable, must not be accomplished prior to evaluator arrival. The agreed upon dates and times for these demonstrations are included at the end of this letter.

Current rosters must be presented identifying the individuals that will maintain around the clock operation at all evaluated facilities.
Demonstration of an actual shift change is not required. State EOC, Dose Assessment Leader, Field Team Coordinator, Radiological Field Monitoring Teams, Joint Public Information Center, Forward Command Post, Callaway CountylFulton EOC, Gasconade County EOC, Montgomery County EOC, Osage County EOC, and the Radiological Officer and Shelter Manager at the Hermann Reception and Care Center shall demonstrate their ability to transition from an outgoing shift to an incoming shift without discontinuity in operations. This will be accomplished by having personnel in key positions provide a shift change briefing either to the evaluators or their actual replacements prior to termination of the exercise.
Criterion lbl - Facilities: All facilities must be fully set up as they would be to support an actual emergency. This would include the availability of operational telephones and other equipment as required for response to an emergency.
Criterion ldl - Communications: Communications must be demonstrated by all facilities and field teams evaluated. The evaluators will request copies of all message and logs of message traffic at each site.
Criterion leI - Equipment and Supplies to Support Operations: Verification of stored KI supplies at all county EOCs and at the State EOC will be accomplished during the exercise.
Criterion 3et - Implementation of Protective Actions for Special Populations - EOCs :
Telephone calls to special facilities and individuals with special needs may be actually made or simulated. Actual telephone calls must be made to at least 113 ofthe transportation providers, including special resources for disabled individuals, during this exercise. However, all facilities, individuals with special needs, and transportation providers required to be notified must be clearly identified and the actual or simulated contacts appropriately documented.
Telecommunications Device for the Deaf calls to the hearing impaired population will be simulated and appropriately documented.
Criterion 3c2 - Implementation of Protective Actions for Special Populations -Schools: We will expect the capability to evacuate schools to be demonstrated by Fulton Public School District #58 and the Osage R-I School District. An exercise evaluator will be assigned to interview the district superintendent (or other designated school official) and principal and at least one school bus driver for each school district. The bus drivers must have dosimetry, a dosimeter charger, and KI available for this demonstration. The interviews described above will be conducted out of sequence from the exercise.
During the emergency phase of the actual exercise, all appropriate actions (e.g., notifications, EAS messages, etc.) must be taken for any public or private schools or day care facilities affected by protective action recommendations. Telephone calls must be actually made and all actual contacts should be appropriately documented.

Criterion 3dl - Implementation of Traffic and Access Control: Deployment of traffic and access control personnel to assigned locations will be simulated. However, the locations where traffic and access control would be established must be appropriately documented. Staffing of traffic and access control points must be appropriately coordinated with all involved jurisdictions. At least two individuals who would perform traffic and access control must be available at the Callaway, Gasconade, Montgomery, and Osage County EOCs for an interview to demonstrate knowledge of their roles and responsibilities concerning traffic and access control, as well as appropriate knowledge concerning dosimetry and potassium iodide procedures. We recommend this demonstration occur early in the exercise.
Criterion 4a3 - Plume Phase Field Measurements and Analyses - Measurements: Each of the deployed field monitoring teams must take radiation measurements at a sufficient number of locations to identify the plume.
Criterion Sal - Activation of the Prompt Alert and Notification System - Timely: An evaluator will be assigned at the EAS station (KTXY) to observe the station's procedures for broadcasting the messages. We expect to see actual receipt of the messages from the appropriate County EOC or the State EOC. Following receipt at the station, procedures to broadcast the message must be fully demonstrated up to the point oftransmission. Actual broadcast ofthe messages or EAS test messages are not required. The FEMA evaluator will remain at the EAS station until the termination of the exercise to observe receipt and broadcast procedures for all EAS messages and Public Information messages. Copies of all EAS messages and Public Information messages will be requested from the facility.
Actual contact with the EAS station and dissemination ofthe initial message to the radio station must be demonstrated. During the Extent of Play meeting it was determined that the State EOC would be responsible for the Initial EAS message with coordination by the County EOCs.
The procedures for siren activation must be demonstrated up to the point of actual activation.
Actual siren activation may be simulated. In addition, tone alert radio and weather radio activation may be simulated by the EAS station, Radio station KTXY.
Criterion Sbl - Emergency Information and Instructions for the Public and the Media:
Media briefings will be conducted at the Joint Public Information Center located at the State Emergency Operations Center in Jefferson City. Rumor control staff at the Joint Public Information Center must demonstrate the capability to respond to an average of at least six calls per hour per operator throughout the emergency phase. The staff should demonstrate the capability to monitor the contents of broadcast media coverage ofthe emergency situation. Any trends in rumors must be addressed in news releases or media briefings. At least one message should address a false or misleading rumor for which measures should be taken.
Criterion 6al - Monitoring and Decontamination of Evacuees and Emergency Workers, and Registration of Evacuees: At the Hermann Reception and Care Center on August 14, 2001 at 0930, the number of evacuees required to be monitored within twelve hours is 54 (20% of the evacuees assigned to the center). The plans indicate monitoring will be performed by a pool of 26
monitors from the following agencies: MoNET, MoNG, and State Highway and Transportation Department. According to the plan, one portal monitor will be available at this facility that will require one monitor. At least six simulated evacuees must be processed into the reception center to allow demonstration of monitoring, decontamination, and registration capabilities. The monitoring sequences for each simulated evacuee will be timed by the evaluators to ensure that all evacuees assigned to this center can be monitored within 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />. Also, a minimum of2 monitors, based upon the plans, must be available with hand held instruments (portal monitor backup) at this facility.
In addition, two monitors (male and female) must be available for evacuee remonitoring following decontamination and 1 monitor for vehicle monitoring. Based upon the above, a total of 6 monitors will be required for this demonstration of monitoring.
The facilities at the Hermann Reception and Care Center, utilized for monitoring and decontamination of evacuees, must be fully activated and setup to receive evacuees for evaluation. Monitoring and decontamination procedures must be initiated for at least one male and one female evacuee. The personnel may simulate decontamination procedures.
All organizations who, per the plan, provide resources in support of registration center activities, must be present for evaluation at the Hermann Reception and Care Center. These organizations are the Division of Family Services, American Red Cross, Missouri Nuclear Emergency Team, monitors from the monitoring pool, and the Herman Police Department.
Criterion 6bl-Monitoring and Decontamination of Emergency Worker Equipment:
Monitoring of at least one emergency worker vehicle must be demonstrated.
Criterion 6c1 - Temporary Care of Evacuees: This will be demonstrated at the Hermann Senior High School. Successful demonstration will fulfill the baseline requirements for evaluation of this facility. Demonstration will not be required again unless there is a significant change made to the facility that would affect the ability to care for evacuees as stated under the current plan.
According to the plans, the Division of Family Services and the American Red Cross provide resources and support for the congregate care center. Therefore, a representative from each of these organizations, must be present during the evaluation.
Criterion 6dl - Transportation and Treatment of Contaminated injured individuals: This criterion is to be demonstrated by the Callaway County Ambulance District and the Callaway Community Hospital on October 2, 2001 at 1000.
In the event that, during an out-of-sequence demonstration an evaluator identifies an exercise issue, the evaluator will discuss it with the Team Leader, Controller, and Trainer (State representative). If possible, the trainer will provide immediate instruction and are-demonstration will occur to correct the issue. The exercise report will reflect the exercise issue and that it has been corrected.

Appendix 4 - Exercise Scenario This appendix contains a summary of the simulated sequence of events -- Exercise Scenario --
which was used as the basis for invoking emergency response actions by OROs during the Callaway Nuclear Power Plant full-scale exercise conducted on October 3,2001.
The times listed below are those contained in the scenario. Actual times of key events are documented in Table 1 of the Exercise Timeline.
The exercise scenario was initially submitted by the Ameren Union Electric Company on August 16, 2001, and, following amendments, was approved by FEMA on August 20, 2001.
The scenario was based on a loss of containment accident resulting in a direct release to the environment.
