LIC-09-0012 Attachment Page 3 I. MANAGEMENT ACTIONS A. Four contractor/vendor employees tested positive for marijuana on a pre-access test. One of the four also tested positive for opiates. All four individuals were denied unescorted access.

B. A contractor/vendor employee refused a random test. The individual left site after being notified that refusal to test is considered a positive result. The individual's unescorted access was revoked and permanently denied.

C. A contractor/vendor employee provided a cold specimen on a pre-access test and refused to provide a second specimen under direct observation. The individual's unescorted access was permanently denied.

D. A contractor/vendor employee tested in the range of 0.02% to 0.04% blood alcohol content (BAC) on a random test. Although not considered a positive test, per Omaha Public Power District procedure, the individual was turned over to their employer and sent home for the day.

E. A contractor/vendor employee tested positive for both amphetamine and methamphetamine on a pre-access test. The individual was permanently denied unescorted access.
