Response

to Notice of Violation Ins ection Re ort 89-10 VIOLATION!

During the Nuclear Regulatory Commission inspection conducted on March 12 April 10,

1989, a violation of NRC requirements was identified.

The violation involved failure to follow maintenance procedures in installing the Unit 2 containment maintenance hatch.

In accordance with the "General Statement of Policy and Procedures for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1988), the violation is listed below:

Unit 2 Technical Specification (TS) 6.8.l.a requires that written procedures shall be established, implemented and maintained covering the activities recommended in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978.

Appendix A, paragraph 9.a.,

includes maintenance that can affect the performance of safety-related equipment.

Paragraph 3.f includes procedures for maintaining integrity of the containment system.

Maintenance procedure M-0311, Rev.

5, Equipment Access Hatch Opening and Closing, specified a standard x-pattern for torque application in two passes, with the intermediate and final torque values specified.

Maintenance procedure M-0039, Rev.

6, Threaded Fasteners of Closure Connections on Pressure Boundaries and Structural Steel, defined the standard x-pattern for the 12 closure bolts.

Contrary to the above, the personnel installing the hatch did not use a standard x-pattern for any pass.

One bolt was also totally missed during the intermediate torque setting.

RESPONSE.

1.

Florida Power

& Light agrees with the violation as stated.

2.

The reason for. the violation was failure to follow procedure.

3.

The Unit 2

containment maintenance hatch was retorqued to specification following the procedure.

4.

Additional training willbe provided to maintenance personnel.

The additional training is scheduled to be completed by August
