The cause is not knova, but suspected to be a soil contact on one NIS channd Following detailed testing of the circuits and repair of a reactor coolant system valve, the unit returned to power operation.

3 250-85-23 EB CENERA A reactor trip occurred due to failure of an instrument inverter which caused the "A" steam generator level to in-crease to the High High levelset point.
This t. ripped the turbine which resulted in a reactor trip. The inverter was repaired and later replaced.

In accordance with requirements of Technical Specification 6.9.1.6, there were challenges to PORV or safety valves during the report month as follows:
- Loss of power to the Engineered Safety Feature Actuation Signal System Relays caused Safety Injection Containment Isolation and !!ain Steam Isolation resulting in a Reactor Trip.
Reactor Coolant System pressure increase actuated the pressurizer Power Operated Relief Valve for less than one second.

- PORV actuations were caused by manually controlling the Reactor Coolant System pressure near the PORV setpoint due to Personnel error. See Event #11
