4 ATTACHMENT Re: Turkey Point Units 3 and 4 Docket Nos. 50-250 & 50-251 Proposed Civil Penalty EA 83-49 Notice of Violation FINDING:

Technical Specification 3.8.4 requires two operable AFW pumps and associated flow paths when the reactor is above 350 degrees F.

Contrary to the above, with Unit 3 in power operation, all AFW pumps were found inoperable on April 19, 1983, at approximately 6:30 a.m. A contributing factor to this violation was a retagging operation on April 14,1983, of two AFW steam supply valves which was accomplished without independent verification, as required by Procedure AP 0103.4, "In Plant Equipment Clearance Orders," Step 4.8 and Figure 3, which implements Technical Specification 6.8.l.

RESPONSE

1.

FPL concurs with the finding.

The reasons for the finding are discussed in detail in your inspection Report 50-250/83-15 and 251/83-15.

The corrective action taken was to open the valves, verify AFW system alignment, and test run AFW pumps B and C. Additional corrective etion is discussed in our letter L-83-249 dated April 21,1983, and in our letter PNS-LI-83-321-1 dated May 3,1983.

The corrective action to prevent recurrence was also discussed in Inspection i

Report 50-250/83-15 and 50-251/83-15.

4 As requested by your August 15, 1983 cover letter to the Notice of Violation, additional corrective action taken to prevent recurrence was to:

a) Revise the nuclear turbine operator logsheets to upgrade the instructions for verifying the auxiliary feedwater flowpaths, b)

Submit a proposed change to Technical Specifications to perform monthly walkdowns of safety systems flowpaths. (See our letter L-83-302, dated May 13,1983.)

c)

Institute a program of monthly walkdowns of safety systems flowpaths to implement the proposed Technical Specifications.

d)

Form a task force at the plant site to review and upgrade specific equipment control areas related to the finding such cs locked valves, key j

control, and equipment labeling.

e) initiate a program of periodic meetings with all operating shifts to emphasize procedure adherence and the importance of fully and correctly performing any jobs assigned.

i f)

Form a task force ut our headquarters to examine areas including quality assurance, security, licensing and engineering and upgrade our capability to identify potential problems in these areas.

Full compliance was achieved on April 19,1983, when the Auxiliary Feedwater System was restored to normal.
