APPENDIX NOTICE OF VIOLATION Gulf States Utilities Docket: 50-458

. River Bend Station Operating License: NPF-47 During an NRC ~ inspection conducted on November 22 through December 31, 1987, one violation of NRC requirements was identified. The violation involved a failure to follow procedure for control of locked valves. In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions,"

10 CFR Part 2, Appendix C (1987), the violation is listed below:

Failure to Follow Procedure for Control of Locked Valves River Bend Station Technical Specifications, paragraph 6.8.1.a. requires that procedures for activities identified in Appendix A of Regulatory

' Guide 1.33,~ Revision 2, February 1978, be established, implemented and maintained. Paragraph 1.c to Regulatory Guide 1.33 requires that administrative controls for equipment control (e.g. , locking and tagging)

be covered by written procedure Operations Support Procedure OSP-0014 "Control of Locked Valves and Devices," requires that when it is necessary to change the position of a locked valve or device,.the authorized individual will make appropriate entries in the Locked Valves / Device Manipulation Log and obtain a locked valve key from the shift supervisor or control operating foreman.
