NRC INSPECTION REPORT 030-04041/97007(DNMS)

Dear Mr. Banfield:

This refers to the special inspections conducted by Region lli on November 19,1997, at the Rochester Hills, Michigan, field office, and by Region I on December 8,1997, at a field site in Trainer, Pennsylvania. The purpose of these inspections was to review the circumstances regarding two drive cable failure / source disconnect events that occurred on November 16,1997, and December 8,1997. The November 19,1997 inspection was prompted by your notification of a 1530 millirem (15.3 mSv) exposure to an individual who performed the source recovery for the November 16,1997 event. The December 8,1997 inspection was prompted by a reciprocity request (received by Region I) by a service company to perform a source retrieval.

The November 19,1997 inspection included continuing in-office review through February 10, 1998, to evaluate the results of the device manufacturer's preliminary metallurgical analysis of the failed drive cable and review additional information provided by you regarding personnel exposure calculations. This also refers to a subsequent telephone exit conference conducted on February 10,1998, between Messrs. Darrel Wiedeman and John Madera and you and Mr. Doug Culver to discuss the results of the inspection and obtain additionalinformation on your source l

retrieval operations.

The enclosed inspection report identifies areas examined during the inspections. Within these areas, the inspections consisted of a selective examination of procedures and representative records, observations, independent measurements, and interviews with personnel.

The matter regarding the drive cable failures is an unresolved issue at this time. As you are aware, the NRC is considering the generic implications in these drive cable failures and will review this matter further.

No violations of NRC requirements were identified during the course of these inspections.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practices," a copy of this letter and its enclosure will be placed in the NRC Public Document Room.
