Appendix h0TICE OF VIOLATION lllinois Power Company Docket No. 50-461 As a result of the inspection conducted on September 9 through October 19, 1985, and in accordance with the General Policy and Procedures for NRC Enforcement Actions (10 CFR Part 2, Appendix C), the following violation was identified:

10 CFR 50, Appendix B, Criterion 11 requires the establishment of a Quality Assurance Program. The Illinois Power (IP) Operational Quality Assurance (QA)

Manual for Clinton Power Station (CPS) states in part, "...this manual...becomes efft.:+ive upon turnover of a system structure or component from the constructor to IP...".

The Fuel Building was officially turned over to IP on August 26, 1985.

The QA manual also states in part, "The IP Nuclear Power Operational QA Program applies to... fire protection..." The manual further states in part, " Appendix B details the scope of its application with respect to activities associated i

with Fire Protection..."

Appendix B of the Operational QA Manual indicates that Chapter 5 of the manual applies to fire protection. Chapter 5 of the manual states in part,

"a. Instructions, Procedures,...shall include appropriate qualitative and/or quantitative acceptance criteria for determining that important activities have been satisfactorily accomplished.

b. Instructions, Procedures,...for installation, testing, and operation shall contain step-by-step instructions in the degree of detail necessary for perfonning a required function or task."

Contrary to the above, the licensee failed to properly calibrate the fire hose standpipe pressure restricting valves located in the Fuel Building, and failed to inspect the portable fire extinguishers also located in the fuel Building in accordance with the establisheri monthly procedure.

This is a Severity Level IV violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office withir t.iety days of the date of this Notice a written statement or explaution in reply, including for each item of noncompliance:

(1) corrective action taken and the.esults achieved; (2) corrective action to be taken to

avoid further noncompliance; and (3) the date when full compliance will be

achieved. Consideration may be given to extending your response time for good cause shown.
