APPENDIX
NOTICE OF VIOLATION Commonwealth Edison Company Docket No. 50-454 As a result of the inspection conducted on September 4 - October 1,1985, and in accordance with the General Policy and Procedure for NRC Enforcement

Actions, (10 CFR Part 2, Appen' dix C), the following violations were identified:
e 1.
Technical Specification 4.2.1.1.la(2) states, in part:
"The indicated AFD shall be determined to be within its limits during power OPERATION...by... monitoring the indicated AFD of.each OPERABLE excore channel...at least once per hour for the first 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> after i
restoring the AFD Monitor Alarm to OPERABLE status."
Contrary to the above, on August 9 - 10, 1985, while in Mode 1, the i
indicated AFD was not monitored hourly for 10 hours1.157407e-4 days <br />0.00278 hours <br />1.653439e-5 weeks <br />3.805e-6 months <br /> following the restoration to OPERABLE status of the AFD Monitor Alarm.

Technical Specification 4.0.2 required that each surveillance requirement shall be performed within the specified time interval with a maximum allowable extension not to exceed 25% of the surveillance interval.

Technical Specification 4.4.6.2.1.d required that a Reactor Coolant System water inventory balance be performed at least once per 72 l
hours.
t Contrary to the above, on August 20, 1985, while in Mode 1, the Reactor Coolant Systen water inventory balance was not perforned within the required surveillance interval.
This is a Severity Level IV violation (Supplement I).
