'During an NRC' inspection conducted during the period of August 16 through
September 15, 1985, a violation of NRC requirements was identified. The
!
violation involved the' failure to follow an administrative procedure for j
control and use of. procedures.
In accordance with the " General Statement of j
Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the violation is listed below-A.
Failure to Follow an Administrative Procedure
.
10 CFR Part 50, Appendix B, Criterion V, and the licensee's approved l
quality assurance program, require that activities affecting the quality of safety-related systems and components be conducted in accordance with
,
j approved instructions, procedures, and drawings.
Administrative Procedure ADM-003, " Development, Control and Use of j
Procedures," Revision 6, requires the following:
i In the event a procedure cannot be followed exactly, the task or
.
i evolution shall not be conducted unless required by an emergency or casualty situation, until the procedure has been revised.
j The initiator of a Temporary Change Notice (TCN) shall specify the i
.
reason for the change on the TCN form.
The initiator shall retain a copy of the TCN for the continuation of
.
the work activity.

j Contrary to the above, ADM-003 procedure requirements were not fully
j implemented for surveillance test procedures STP-503-4203 (completed l
j 8/02/85, STP-503-4204 (completed 8/03/85), STP-503-4205 (completed
}
8/06/85, and STP-503-4206 (completed 8/02/85) for the semiannual l
,
calibration of source range monitor channels A, B, C, and 0 in that the i
j STPs were not followed exactly. TCNs 85-1405 and 85-1406 were issued and
'
approved without the reason for the change being specified on the TCN form f
and TCNs 85-1436, 1214, 1440, 1453, and 1454 were not fully implemented
j for the continuation of the work activity. Nur,1erous examples
!
of the failure to follow STP procedures exactly and the failure to fully
'
implement TCNs were documented.

transmitting -this ' Notice, a written statement or explanation in reply, including for each violation:
(1) the reason for.the violations if admitted, (2) the corrective steps which have been taken and the results achieved,-(3)

the' corrective steps which will be taken to avoid further violations, and (4)
the_date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
