APPENDIX NOTICE OF VIOLATION Commonwealth Edison Company Docket No. 50-454 As a result of the inspection conducted on June 4 - July 1, 1985, and in accordance with the General Policy and Procedures for NRC Enforcement Actions, (10 CFR Part 2, Appendix C), the following violation was identified:
Technical Specification 4.0.3 states:
" Failure to perform a surveillance requirement within the specified time interval shall constitute a failure to meet the OPERABILITY requirements for a Limiting Condition for Operation."
Technical Specification 4.0.4 states, in part:
" Entry into an OPERATIONAL MODE...shall not be made unless the surveillance requirement (s) associated with the limiting condition for operation have been performed within the stated surveillance interval..."
Technical Specification 4.2.5 requires that Indicated Reactor Coolant System Average Temperature (T

Contrary to the above:
a.
From March 28 to May 5, 1985, while in Mode 1, T and P were ave pzr not verified within their limits.
b.
On March 29 and April 3, 13,' 16 and 21, 1985, Mode 1 was entered without verifying T and P were within their limits.
ave pzr This is a Severity Level IV violation (Supplement I). (454/85025-02(DRP))
The inspection showed that action had been taken to correct-the identified violation and to prevent recurrence.
Consequently, no "eply to this violation is required and we have no further questions regarding this matter.
