APPENDIX NOTICE OF VIOLATION Commonwealth Edison Company Docket No. 50-454 As a result of the inspection conducted on June 4 - July 1, 1985, and in accordance with the General Policy and Procedures for NRC Enforcement Actions, (10 CFR Part 2, Appendix C), the following violation was identified:

Technical Specification 4.0.3 states: " Failure to perform a surveillance requirement within the specified time interval shall constitute a failure to meet the OPERABILITY requirements for a Limiting Condition for Operation."

Technical Specification 4.0.4 states, in part: " Entry into an OPERATIONAL MODE...shall not be made unless the surveillance requirement (s) associated with the limiting condition for operation have been performed within the stated surveillance interval..."

Technical Specification 4.2.5 requires that Indicated Reactor Coolant System Average Temperature (T a ) and Indicated Pressurizer Pressure (P ho@[) .

be verified within the dmits of Table 3.2-1 at least once per 12 Contrary to the above: From March 28 to May 5, 1985, while in Mode 1, Tave and P pzr were not verified within their limit On March 29 and April 3, 13,' 16 and 21, 1985, Mode 1 was entered without verifying T ave and P pzr were within their limit This is a Severity Level IV violation (Supplement I).
