Examination of Geologic Anomaly The IE inspectors accompanied the Chief of the Geosciences Branch, Division of Site Safety & Environmental Analysis, NRR, during his examination of soil anomalies which were discovered during trenching in the Unit 1 foundation area. His preliminary findings were that the anomalies appeared to be depositional features and were not of tectonic origi The applicant was requested to provide the results of their current mapping activities and a summary report with their conclusions to NRR who will schedule another site visit to further review this matter after the maps and report are submitted. The applicant was requested to leave the trenches undisturbed until after the next NRR site visit which is expected to occur in approximately two week No deviations were identifie . Review of Foundation Bearing Capacity Verification Procedure Black & Veatch (B&V) Procedure No. PS0-01-013178, Revision 0, "Exca-vation Mapping, Inspection, and Monitoring Procedures," was reviewed by the IE inspector. Paragraph 3.4.1 requires that all Category I foundation excavations be inspected to verify that the specified bearing capacity is present at the foundation bearing surfac . _ - . . . . .__

For Savanna Shale and Siltstone, the procedure requires in-place density determinations performed in accordance with ASTM D1556-74, ,

" Standard Test Method for Density of Soil In Place By The Sand-Cone Method." Section 1 of ASTM 1556-74 states, "The apparatus described herein is restricted to tests in soils containing particles not larger than 2 inches (50.8 mm) in diameter." The applicability of the restriction to the Savanna Shale and Siltstone was discussed with the licensee representatives who acknowledged the discrepanc Alternate methods of verifying the in-place bearing capacity will be identified by revision to the procedur The B&V procedure also states that for overburden or highly weathered Boggy or Savanna Formations, the testing will consist of running Torvane Shear Device tests. The Torvane is a device considered as a soil classification instrument by its manufacturer, Soiltest, Inc.,

and as such, its use for the determination of bearing capacity depends on agreement between the Unconfined Compression Test and the Torvane test. This relationship could not be identified during this inspectio The in-place verification of the bearing capacities of foundation soils is considered an unresolved item which will be reviewed during a subse-quent inspectio . Quality Assurance Surveillance PS0 Quality Field Procedure No. 8.01, Revision 0,' " Quality Assurance Construction Site Surveillance," was reviewed by the IE inspecto This procedure delineates the methodology for the performance of sur-veillance activities by the Quality Assurance grou Twenty-two Surveillance Checklist and Surveillance Report forms documenting the results of surveillance of non-Category I work were reviewed. The documented implementation was in accordance with the procedure and was performed by the QA-Civil Engineer for familiariza-tion with the procedure before working to Category I criteri No deviations were identified during this portion of the inspectio . . Exit Interview

- The IE inspector met with applicant representatives (denoted in para-graph 1) at the conclusion of the inspection on October 11, 1978. The IE inspector summarized the purpose and scope of the inspection and the findings. The applicant representatives acknowledged the unresolved item and indicated that steps to resolve the matter had already been initiate .
