I & E Inspection Report 83-09 REFERENCE:
NRC Letter dated May 14, 1983

Dear Mr. Collins:
This letter is in response to the Notice of Violation received as a result of inspections conducted at Fort St. Vrain during the period of April 11-15, 1983. Tne following response to the items contained in the Notice of Violation is hereby submitted:
1.
Technical Specification 7.4.a,
" Procedures, Administrative Controls," states, in part, that "... written procedures shall be established, implemented, and maintained...."
Licensee Procedure QCIM-1, " Calibration of Measuring and Test Equipment,"
Issue 2, effective date October 13,-1981, states, in part,
"3.2.
A notification system shall be established which will ensure that equipment due for calibration shall be identified and calibrated at the proper intervals.
"4.1.4 Upon'
completion of a
calibretion the Maintenance / Calibration History Card will be completed."
Contrary to the above, no notification system had been established as required in Section 3.2 and none of the maintenance / calibration history cards had been completed as required by Section 4.1.4.

(1) The corrective steps which have been taken and the results achieved:
All maintenance / history cards have been reviewed and all calibrations brought up to date.
The cards have been initialed.
Full compliance was achieved on April 11, 1983.
(2) Corrective steps which will be taken to avoid further violations:
Procedure QCIM-1 has been revised to include a requirement for a quarterly review of the maintenance / history card system with a provision being made for initialing the card at each review.
This review is the responsibility of the QA/QC Supervisor.
(3) The date when full compliance will be achieved:
Revisions to QCIM-1 and final approvals will be completed by June 17, 1983.

Administrative Procedure Q-12, " Control of Measuring and Test Equipment,"
Issue 4, effective date March 17, 1982, states, in part,

"4.2 Calibration Procedures Calibration procedures are required for all calibrations...
.
The procedures are required to provide:
a) Step by step instructions for performing the calibration."
Contrary to the above, no calibration procedure existed for the calibration of Thermocouples T1-T6.
This is a
Severity Level IV Violation.

-3-(1) The corrective steps which have been taken and the results achieved:
Thermocouples T1*T6 are being removed from calibration service.
Thermocouples T1-T6 will then be calibrated by an outside company approved vendor.
Based on the test results, an evaluation as to the acceptability of data from them will be performed.
(2) Corrective steps which will be taken to avoid further violations:
Thermocouple standards will be-calibrated off-site by approved outside NBS traceable vendors.
(3) The date when' full compliance will be achieved:
September 1, 1983.
