These numbers were called 11arrival survey numbers" (AS No.) and were used by the inspectors to identify licensee-shippers during this inspectio.

Package Integrity The shipment from Surry carried by Chem-Nuclear Systems, Inc., (Tractor #19)

identified by AS #24988 was found to. be in violation of_ 49 CFR 173.392(c)(l)

in that. there were holes in the bottcim and front of one dumpster.. This is. a *.
