ATTN: Anthvny Spencer President P. 3. Box 9858 Tulsa, Oklahoma 74157 Gentlemen:
This refers to the routine, unannounced radiation safety inspection condutted by Messrs. A. D. Gaines and D. A, Powers of this office on June 28, 1988, and our meeting with you on July 11, 1988, of the activities authorized by Nebraska Byproduct Material Licens 9 -41-01 and to the discussion of our findings held by the inspectors with you at the conclusion of the inspection.
The inspection was an examination of the activities conducted under the license as they relate to radiation safety end to compliance with the Commission's rules and ragulat!ons and the conditions of the license.
The inspection consisted of selective examinations of procedures and representative records, interviews of personnel, independent neasurements, and observations by the inspectors.
During this inspectict., ccrtain of your activities were found not to be conducted in full compliance with NRC requirements.
Due to the fact that the violation noted in the Notice of Violation cetterns working in a nonagreement state without a general or specific license, and Sierra Testing, Inc. has applied for and has received NRC Byproduct haterial License 35-26950-01, no response to this matter is needed.
Please note, some apparent violations that were discussed with you at the conclusion of the inspection have been reevaluated and subsequently withdrawn, so they do not appear as violations in the attached Notice of Violation.
