NRC INSPECTION REPORT 150-00017/93-02 This refers to the routine, unannounced inspection conducted by.

Mr. Mark R. Shaffer of this office on December 2,1993. The inspection included a review of licensed activities in NRC jurisdiction pursuant to the general license authorized by NRC in 10 CFR 150.20. The general license authorizes the Agreement State licensee to perform radiography in accordance'

with the terms and conditions specified in License LA-6012-L01 issued by the

State of Louisiana and with the NRC regulations listed in 10 CFR 150.20(b).

At the conclusion of the inspection, the findings were discussed with members of your staff.

The inspection was an examination of activities conducted under the license as they relate to radiation' safety and to compliance with the Commission's' rules and regulations and the conditions of the license.

The inspection consisted of selective examinations of procedures and representative records, interviews

of personnel, independent measurements, and observation of activities in progress.

No violations or deviations were identified; therefore, no response to this letter is required.

This inspection consisted of a field site inspection at one of the licensee's temporary jobsites in Guernsey, Wyoming, to observe radiographic operations.

Two radiographers.were inspecting welds on a pipeline project using an exposure device containing approximately 54 curies of iridium-192. The inspector observed the radiographers perform approximately 12 exposures.

It was noted that the radiographers conducted required surveys of the exposure device and adequately performed evaluations necessary to establish a restricted boundary around the work area. The -inspector ~also performed independent radiation measurements to verify compliance with NRC regulations.

The radiographic exposure device was noted to have the appropriate labels in accordance with 10 CFR 20.203(f), and exposure levels at the surface of these devices were within the limits specified by-10 CFR 34.21(a).

The radiography crew possessed two calibrated and operable survey instruments l

which had a range of 0 to 1,000 millirems per hour.

-2-i wearing a direct reading pocket dosimeter, a thermoluminescent dosimeter, and an alarm ratemeter.

The pocket dosimeters had an adequate range of 0 to 200 milliroentgens.

By directly reading the devices and through discussions with the radiographers, the inspector observed that pocket dosimeters had been recharged _ at the start of each shift as required by 10 CFR 34.33(a).

The radiographic exposure device had been transported to the temporary job site in the licensee's private vehicle.

The radiation levels of the package were such that RADI0 ACTIVE YELLOW-II labels were appropriately affixed to the package. Therefore, the licensee's vehicle was not required to be placarded.

The inspector reviewed the shipping papers associated with the package to verify compliance with 49 CFR 172.200-204.

The description of the hazardous material (iridium-192) on the shipping papers included all required information (e.g., name of the radionuclide, the physical form of the material, the activity contained in the package, the category of label assigned, and the transport index).

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter will be placed in the NRC Public Document Room.
