Attachment I, the Steam Generator Tube Inspection Report, is being submitted in accordance with Wolf Creek Generating Station (WCGS) Technical Specification (TS) 5.6.10, "Steam Generator Tube Inspection Report." This report provides the results of WCGS's fifteenth steam generator (SG) tube inservice inspection. The Steam Generator Tube Inspection Report includes the following:

a. The scope of inspections performed on each SG;
b. Active degradation mechanisms found;
c. Nondestructive examination techniques utilized for each degradation mechanism;
d. Location, orientation (if linear), and measured sizes (if available) of service, induced indications;
e. Number of tubes plugged during the inspection outage for each active degradation mechanism;
f. Total number and percentage of tubes plugged to date; and
g. The results of condition monitoring, including the results of tube pulls and in-situ testing.

The following items are also included in accordance with the Interim Alternate Repair.Criteria (IARC), which was approved to support WCGS's Refueling Outage 16:

h. Following completion of an inspection performed in Refueling Outage 16 (and any inspections performed in the subsequent operating cycle), the number of indications and location, size, orientation, whether initiated on primary or secondary side for each service-induced flaw within the thickness of the tubesheet, and the total of the circumferential components and any circumferential overlap below 17 inches from the top of the tubesheet as determined in accordance with TS 5.5.9c.1; P.O. Following completion of an inspection; performed in Refueling Outage 16.(and any inspections performed in the subsequent operating cycle), the primary to secondary LEAKAGE rate observed in each SG (if it is not practical to assign leakage to'an individual SG, the entire primary to secondary LEAKAGE should be conservatively assumed to be from one SG) during the cycle preceding the inspection which is the subject of the report; and
j. Following completion of an inspection performed in Refueling Outage 16 (and any inspections performed in the subsequent operating cycle), the calculated accident leakage rate from the portion of the tube below 17 inches from the top of the tubesheet for the most limiting accident in the most limiting SG.

Enclosures 1-5 contain the service-induced indications and flaws within the.thickness of the tubesheet identified during WCGS's Refueling Outage 16.

Attachment I to ET 08-0047 Page 1 of 7 STEAM GENERATOR TUBE INSPECTION REPORT This report is being submitted pursuant to Technical Specification (TS) 5.6.10, "Steam Generator Tube Ihspection Report," to report the results of the fifteenth steam generator (SG) tube inservice inspection.

Summary The fifteenth SG tube inservice inspection was completed by Westinghouse Electric Corporation on April 14, 2008, during Refueling Outage 16. Initial entry into MODE 4 was achieved on May 7, 2008.

Refueling Outage 16 was the first inspection of the third sequential inspection interval (60

-effective full power months (EFPMs)). At the start of Refueling Outage 16 the SGs had operated for a total of 230.76 EFPMs.

Permanent resolution to an alternate repair criteria for indications within the tubesheet could not

-beachieved prior to Refueling Outage 16. , As a result, an Interim Alternate Repair Criteria (IARC) was approved which revised TS 5.5.9, "Steam Generator (SG) Program". This approach required an inspection to the full depth of the hot leg tubesheet, but also allowed axial cracks and circumferential cracks less than a specified extent to remain in service at the tube ends. Indications were identified in the tube ends of SGs B and C which required tube end inspection scope expansions into SGs A and D.

Other than the tube end indications, interpreted as primary water stress corrosion cracking (PWSCC), no other corrosion related tube degradation mechanisms have been detected.

Wear at Anti-Vibration Bar (AVB) intersections is an existing damage mechanism.

Secondary side maintenance and inspections were also performed.

The condition of the SGs has met all industry and regulatory structural and leakage integrity requirements.

Section 1 of this report describes the scope of inspections performed, including the nondestructive examination techniques utilized for each degradation mechanism.

Section 2 contains the location, orientation -(if linear), and measured sizes (if available) of service induced indications. Section 2 also contains the number of indications and location, size, orientation, whether initiated on primary or secondary side for each service-induced flaw within the thickness of the tubesheet, and the total of the circumferential components and any circumferential overlap below 17 inches from the top of the tubesheet as determined in accordance with TS 5.5.9c. 1.

Section 3 contains the inspection results, which include:

a. Active degradation mechanisms found;
b. Number of tubes plugged during the inspection outage for each active degradation mechanism;
c. Total number and percentage of tubes plugged to date;
d. The results of condition monitoring, including the results of tube pulls and in-situ testing;
e. The primary to secondary leakage rate observed in each SG; and

f. The calculated accident leakage rate from the portion of the tube below 17 inches from the top of the tubesheet for the most limiting accident in the most limiting SG.

Section 1: Scope and Examination Techniques of Inspections Performed The actual inspections performed during Refueling Outage 16 exceeded both the TS minimum requirements and the requirements of the Electric Power Research Institute (EPRI) "Steam Generator Management Program: Pressurized Water Reactor (PWR) Steam Generator Examination Guidelines." The following inspections were initially performed in SGs B and C, and due to tube end indications, the scope of inspections was expanded to the tube ends in SGs A and D.

  • 100% bobbin coil examination of the tubes full length with the exception of the U-bend portion of the row 1 and row 2 tubes
  • 50% Plus Point examination of the row 1 and row 2 U-bends
  • 55% Plus Point examination of the hot leg top of tubesheet +3/-17", including 100% of the peripheral tubes, 2 pitches into the bundle
  • 100% Plus Point examination of the cold leg peripheral tubes top of tubesheet +/-3", 2 pitches into the bundle
  • 20% Plus Point examination of the hot leg tubesheet full depth o As a result of this inspection, indications which exceeded the IARC acceptance criteria were found in the bottom 0.2" of the tubing and the scope was expanded in accordance with the EPRI PWR SG Examination Guidelines. In total, 100% of the SG B, C, and D hot leg tube ends (tube end hot +1") and 30% of SG A tube ends were inspected. (The SG A inspections were concluded at 30% because no indications were detected.)
  • Special Interest Plus Point examinations of all hot and cold leg possible flaw locations including all "I-codes" that were new or not resolved by history review
  • Plus Point examination of all new hot leg (including the U-bend) dings and dents > 5 volts and 50% of existing dents > 5 volts
  • Plus Point examination of a 5% sample of bobbin indications that have not changed since the prior inspection ("H" and "S" codes)
  • Plus Point inspection to bound (all surrounding tubes, at least one pitch removed) the tubes exhibiting possible loose part signals
  • Acquisition of low frequency bobbin data and a sample of Plus Point data to support scale profiling analysis to provide information on the secondary side deposit accumulation
  • Visual inspection of all plugs The initial inspection results and scope expansion plans were discussed in a teleconference on April 5, 2008, between the Nuclear Regulatory Commission (NRC) and Wolf Creek Nuclear Operating Corporation (WCNOC) representatives. The results of that teleconference were subsequently.documented in a letter dated May 8, 2008, from B. K. Singal, USNRC, to R. A.

Muench, WCNOC.

In addition, the following secondary side maintenance and inspections were performed:

  • Upper Bundle In-Bundle visual inspection of SG C

" Top of Tubesheet In-Bundle visual inspections of SGs B and C

" Upper Steam Drum visual inspections of SG B Section 2: Indication Listing The following Enclosures contain the service-induced indications as well as flaws identified within the thickness of the tubesheet. This encompasses the location, orientation (if linear), and measured sizes (if available) of service-induced indications. The Enclosures also contain the number of indications and location, size, orientation, for each service-induced flaw within the thickness of the tubesheet, and the total of the circumferential components and any circumferential overlap below 17 inches from the top of the tubesheet as determined in accordance with TS 5.5.9c.1. Indications identified within the tubesheet were initiated on the primary side.

  • Tube (11, 121) in SG B was administratively plugged. This tube was not previously expanded into the hot leg tubesheet. During the outage, the tube was expanded into the tubesheet by rolling approximately 2 inches from the tube end and installing a mechanical plug. The tube had no history of degradation.

  • An additional 3 plugs are installed in SG A cold leg only, due to tubesheet drilling errors during manufacturing. No tubes are installed in those locations.

Results of Condition Monitoring The examination results confirm that the Wolf Creek Generating Station (WCGS) SGs currently meet all industry and regulatory structural and leakage integrity guidance. No indications were found to exceed structural limits. No tube pulls or in-situ testing were required.

  • AVB wear is an existing degradation mechanism in both SGs B and C.
  • The maximum observed indication (54% through wall depth, AVB wear) meets the structural and leakage requirements.

" Indications interpreted as PWSCC were observed at the hot leg tube ends in SGs B, C, and D; none were observed in SG A. All indications were within about 0.2 inch from the tube end. Indications with circumferential extent greater than 940 were plugged. All axial and circumferentially oriented indications 940 or less in circumferential extent were left in service consistent with the criteria provided in the IARC. Axial indications and indications with circumferential extent of up to, and including, 940 do not challenge the structural and leakage integrity requirements of Nuclear Energy Institute (NEI) 97-06, "Steam Generator Program Guidelines."

  • No other corrosion related degradation mechanisms were observed.
  • No primary to secondary leakage was reported during Cycle 16. No accident induced leakage is predicted from the tubesheet expansion region even if a conservative ratio of accident induced leakage to normal operating leakage of 2.5 were to be applied. Tube burst is prevented by the close proximity of the tubesheet to the expanded tubes. It is concluded that all condition monitoring requirements were satisfied by the condition of the SGs at the end of Cycle 16.
  • No leakage was observed during Cycle 16. Further, no degradation that may lead to leakage was detected above the tubesheet during Refueling Outage 16. Therefore, the entire allowable accident induced leakage (.25 gallons per minute (gpm)) can be allocated to the tubesheet expansion region. The allowable normal accident leakage from the tubesheet expansion region is 0.1 gpm (0.25 gpm divided by 2.5) which is equivalent to the 150 gallons per day (gpd) allowable leakage per WCGS TS 3.4.13, "RCS Operational LEAKAGE." Therefore, no adjustment of the administrative limit for normal operating leakage is required.

" Wear (volumetric) indications detected at the top of the tubesheet have not changed since the prior inspection during Refueling Outage 14 (note 1). One new indication was identified in SG B at the top of the tubesheet on the cold leg side. Three tubes in SG C were identified with volumetric degradation approximately 8-9 inches above the top of the tubesheet. These indications were previously noted (1996), but recorded under a different code, and have not changed since then. The likely source of the volumetric indications at the top of the tubesheet is wear due to a transient foreign object at a prior

Attachment I to ET 08-0047 Page 7 of 7 STEAM GENERATOR TUBE INSPECTION REPORT time. No foreign objects were found among the tubes with volumetric indications and their neighboring tubes. These indications meet the condition monitoring structural requirements.

  • Wear at the flow distribution baffle, attributed to a prior application of pressure pulse cleaning of the SGs, is not a continuing degradation mechanism. No new indications were reported, as expected; all indications were previously reported during Refueling Outage 14. These indications meet the condition monitoring structural requirements.
  • Wear detected at two tube support plate intersections was attributed to a prior transient foreign object and has not changed since the prior inspection during Refueling Outage 14 (note 1). These indications meet the condition monitoring structural requirements.
  • No degradation indications were observed related to potential precursor signals such as dents and dings, or interferences such as permeability variations.
  • No precursor signals were found in tubes identified as potentially having an elevated residual stress condition.
  • No tube-to-tube contact wear was found on any tubes adjacent to tubes that were previously plugged due to AVB wear. Therefore, in accordance with evaluations documented in the Degradation Assessment, stabilization of any previously plugged tubes was not required.
  • The single unexpanded tube (R1l, C121) in SG B was expanded into the tubesheet by rolling approximately 2 inches from the tube end, and a plug was installed. This tube had no history of degradation.
  • Foreign object search and retrieval (FOSAR) activities identified and removed some minor foreign objects. The analysis of the foreign objects that could not be retrieved concluded that they will not limit operation of the SGs for at least the next two plant operating cycles. Possible loose part (PLP) indications reported by eddy current testing were further evaluated by visual inspection and bounded using the Plus Point probe. No degradation was associated with any PLP indication.

" During secondary side visual inspections of the feedwater ring, evidence of minor erosion of the J-nozzles was identified. There are no immediate concerns and no actions were recommended other than continued surveillance at the next scheduled inspection.

" All plugs were found to be in acceptable condition based on visual inspection.

Note 1: Small sizing variations were noted but these are within the variability of the sizing technique.
