| Event Description | The following is a summary of information provided by the Utah Division of Waste Management and Radiation Control (DWMRC) via email:
On February 24, 2026, at approximately 1500 MST, the licensee reported that two fixed gauge shutters were found stuck open during an annual shutter check. The licensee has scheduled a vendor to replace the gauges. The licensee did not provide DWMRC with any information about the device types or radioisotopes involved at the time of the report.
