Title	Agreement State Report - Patient Received More than Prescribed Dose
Event Description	A patient received 150 mCi instead of the 30 mCi of I-131 that was prescribed for thyroid ablation treatment. The cause of the overdose was the patient was misidentified. The patient and prescribing physician has been informed. No adverse health effects are expected. A Medical Event may indicate potential problems in a medical facility's use of radioactive materials. It does not necessarily result in harm to the patient.
