Title	Agreement State Report - Moisture/Density Gauge Shutter Stuck in Open Position
Event Description	The following report was received from the State of Arizona via email: This first notice constitutes early notice of events of possible safety or public interest significance. The information is as initially received without verification or evaluation, and is basically all that is known by the [State of Arizona] Agency Staff at this time. On July 29, 2014, during an inspection, the inspectors noted that a moisture density gauge had a shutter mechanism that was stuck open with the handle in the closed position. The gauge is a Troxler Model 3440, Serial Number 29505, containing 10 millicuries of Cesium-137 and 50 millicuries of Am-241. The licensee is scheduling to have a service company come out to service the gauge. The investigation into this event is ongoing pending the licensee servicing the gauge.
