| Title | Agreement State Report Involving an Unintended Dose to a Fetus |
| Event Description | On 6/26/14 a 23 year old female patient received 100 mCi of I-131 for a thyroid ablation procedure. Prior to treatment, she tested negative for pregnancy on 6/25/14. Subsequent to the procedure, her physician requested a re-test which confirmed that she was pregnant. The estimated date of conception is 6/23/14. The RSO calculated an estimated dose of 25 Rad to the fetus.
This incident was reported to the Florida Bureau of Radiation on 7/22/14. |
