The following occurred at St. Louis University Hospital: A 30 mCi
I-131 Sodium Iodide oral capsule was prescribed for administration to a patient for ablation of residual tissue following a thyroidectomy. However, a 115 mCi
I-131 Sodium Iodide capsule was administered instead, due to conflicting information in the patient record.
At this time, it is believed that there will be no adverse affect to the patient. The patient has been notified of this incident.
The Radiation Safety Officer will be reviewing this incident to determine what actions to take to prevent recurrence.
A Medical Event may indicate potential problems in a medical facility's use of radioactive materials. It does not necessarily result in harm to the patient.
