| Event Description | On 12/09/05 a patient was to receive 220 millicuries of I-131 (three capsules). The three capsules were transported in two small pigs. On Tuesday,12/13/05, one of the capsules was discovered in one of the two pigs indicating the patient only received two of the capsules. The patient will be notified.
