Dear Commissioners and Staff:

Pursuant to 10 CFR 26.719, "Reporting requirements," paragraph (c), "Drug and alcohol testing errors," Pacific Gas & Electric Company (PG&E) is submitting the following report of an investigation into unsatisfactory performance discovered in performance testing associated with the Diablo Canyon Power Plant (DCPP) Fitness For Duty (FFD) Program.

On October 15, 2012, an unsatisfactory performance discovered in testing of a blind performance sample processed at a Health and Human Services-certified laboratory, Laboratory Corporation of America (LC) testing facility, was reported to the DCPP FFD Program Supervisor. A urine specimen spiked with Tetrahydrocannabinol (THC) prepared by a contracted blind performance specimen provider was reported by LC as "negative" when it should have been reported as "positive for THC." LC was notified bf the performance specimen result and its potential performance error. LC has reviewed its records, and the enclosure dated October 18, 2012, documents the outcome of LCIs investigation and corrective action.

PG&E makes no regulatory commitments (as defined by NEI 99-04) in this letter.

Dear Mr. Ryan,

I received correspondence indicating that the result reported to you on a Blind Quality Control specimen was not consistent with the result you expected on the Blind QC sample per your manufacturer's Certificate of Analysis. Laboratory Corporation of America Holdings in Research Triangle Park, North Carolina reported Specimen ,10 Number 0385332356 (Donor ID:CTAW) as negative, yet the expected result was positive for THCA.

This specimen did yield a presumptive positive result on the initial test for THC. It was extracted and analyzed by GC/MS in THe Batch 110231; 0385332356 was the only specimen in the batch and THeA was negative. As part of this investigation, the THeA confirmation was repeated on this specimen; 0385332356 was positive for THeA at 101 ng/mL. A corrected report was issued on October 18, 2012 documenting the change from negative to positive at 101 ng/mL.

Since 0385332356 was the only sample in the original THC batch, an aliquotting issue is a possibility as the source of the initial negative result. Appropriate corrective action 'is being taken by the laboratory.
