Dear Ms. Buffe:
r On June 2,1995, twenty-five (25) blind proficiency urine specimens were forwarded for testing to our approved laboratory, American Medical Laboratories, in accordance with BWNT's 10CFR26 Fitness for Duty Program. On June 7,1995, two (2) of the specimens did not result in the expected cutoff level as specified by the vendor, Drug Testing Consultants, from which we purchased the samples. Both deviations fell 20 ng/ml below the range.
Because 10CFR26 is not definite as to what action is required in such an event, Loren Bush, NRC, was contacted. Documentation regarding this telephone conversation is attached. Upon Mr. Bush's suggestion, BWNT conducted an investigation to determine the cause of the deviation.
Drug Testing Consultants was alerted to the deviation and requested to address possible issues tnat may have caused the problem. American Medical Laboratories was consulted also.
Attached is the documentation from the two companies. (It should be noted that Dr. M. A.
EISohly's letter to Bob Schoening, dated June 16,1995, references " Babcock and Wilcox" incorrectly, this reference is intended to be "BWNT".) The result of this investigation was forwarded to the Document Center. NRC.
Our conclusion concurs that the intent of blind proficiency testing is not to analytically challenge-a laboratory's ability to quantitate drugs and an isolated event of this type is not a grave concern. The investigative documents are enclosed for your review; they will remain on file in BWNT Access Control for audit purposes.
