Crystal River Unit 3 - Report Required by Environmental Protection Plan
Dear Sir:
In accordance with the conditions of the National Pollutant Discharge Elimination System (NPDES) Permit, Progress Energy Florida, Inc. submitted a report to the Florida Department of Environmental Protection (FDEP) on June 2, 2004.
The report summarized the events that occurred on June 1, 2004, when Crystal River Units 1, 2, and 3 experienced an upset condition which resulted in an exceedance of permitted thermal limits.
The attached copy of the written notification made to FDEP is being submitted in accordance with the Crystal River Unit 3 Improved Technical Specification (ITS),
Appendix B, Environmental Protection Plan.

Re: Progress Energy Florida, Inc. Crystal River Units 1, 2, & 3 Upset Thermal Limit Excursion Follow Up Notification - FL0000159 On June 2, 2004, Progress Energy Corporation made a verbal notification of upset to Ms.
Ilia Balcom pursuant to Condition VIH.A.20 of the subject permit. Per her request, the following is.a written description of events that occurred on June 1, 2004 and reported verbally by Mr. Michael Shrader. This event resulted in an upset condition with a subsequent exceedance of permitted thermal limits.
Conditions Prior To Event Immediately prior to the event, the Crystal River South units were generating electricity at or near full capacity (Unit 1 at 400 megawatts (MW), Unit 2 at 475 MNW), and the Point of Discharge (POD) 3 hr rolling average was slightly below 96°F (Permit limitation is 96.50 F).
The intake canal temperature was 87.5°. The ambient temperature was 97.00F.
Event Description At 1430 hours0.0166 days <br />0.397 hours <br />0.00236 weeks <br />5.44115e-4 months <br /> the breaker supplying electrical power to #2 Helper Cooling Tower (HCT) fans and motor operated valves and #1 HCT valves tripped. The loss of power to the.
HCT valves caused HCT pumps #1 and #2 to trip. The electrical supply breaker to the fans and valves tripped as a result of overheating of the air in the room which houses the breaker cabinets. This room is serviced by a temporary 10 ton air conditioning unit.
Immediate actions were taken to reduce electrical generation from both units, while electricians worked to restore the system. From 1440 throughl1540, Unit 1 was reduced in generation from 400 MW to 120 MW, Unit 2 from 475 MW to 140 MW. Power was restored and the HCT's were fully operational at 1540. In the event that subsequent HCT trips might occur and to hasten the reduction of the 3 hour3.472222e-5 days <br />8.333333e-4 hours <br />4.960317e-6 weeks <br />1.1415e-6 months <br /> temperature average, Unit 1 was further reduced in load from 120 MW to 70 MW from 1540 to 1710. At 1610 the 3 hour3.472222e-5 days <br />8.333333e-4 hours <br />4.960317e-6 weeks <br />1.1415e-6 months <br /> rolling average at the POD was 96.65 oF, exceeding the 96.5 "F limitation. Both Units were returned to normal operational conditions. There is no indication that any environmental damage occurred as the result of the upset condition.

To help prevent a reoccurrence of this type of event, additional temporary air conditioning equipment is being installed in the electrical services building where the breaker referenced above is located.
