| Description | TS 5.7.2 requires that areas with radiation levels greater than or equal to 1000 mrem/hr be locked and the keys be maintained under the administrative control of Operations or health physics (HP) supervision. Contrary to this, on March 31, 2008, it was discovered that Security had master keys to Locked High Radiation Areas (LHRAs) that were not under the administrative control of Operations or HP supervision. This violation was discovered by HP supervisors performing an audit of key controls based on recent industry operating experience. Immediate corrective actions were taken upon discovery and documented in CR 2008103892. Although this event involved failure to maintain proper controls to LHRAs, this finding is of very low safety significance because there was no evidence of unauthorized worker entry into the affected areas nor any unexpected radiation exposures to licensee personnel. |
