Failure to Institute Effective Corrective Action to Preclude Repetition

Description

An NRC-identified,Green non-cited violation of 10CFRPart50, AppendixB, CriterionXVI, Corrective Action, was identified when the licensee failed to institute effective corrective actions to preclude repetition of a significant condition adverse to quality. Specifically, the licensee left a secondary containment personnel hatch in an open configuration for approximately 30 minutes while performing a roof inspection, which rendered secondary containment inoperable. This issue had also previously occurred in 2016, but corrective actions to prevent it from occurring again were ineffective.
