TS 3.6.10 required two AVS trains to be
operable in Modes 1, 2, 3 and 4.
TS 3.6.10, Condition B, specified if one or more AVS train pre-heaters was
inoperable, the pre-heater must be restored to
operable status within 7 days or a report must be initiated within 7 days, detailing the reason for the pre-heater inoperability and corrective actions to restore the pre-heater, and submitted to the NRC within 30 days.
TS 3.6.10, Condition C, specified if the required actions and completion times of Condition B were not met, the plant shall be shut down to Mode 3 within

36 hours4.166667e-4 days <br />0.01 hours <br />5.952381e-5 weeks <br />1.3698e-5 months <br />. Contrary to the above, from October 29, 2011, to December 10, 2011, the 2B AVS pre-heater was not
operable and the licensee failed to meet either Condition B or Condition C. This violation is not greater than Green because the degraded pre-heater did not result in a loss of safety function of the 2B AVS. This violation was documented in the licensees corrective action program as
PIP M-11-9216.
