At 0140 EST on March 19,2019, while Unit 2 was at approximately 1 percent power in Mode 2, Operations dedared the High Pressure Coolant Injection (HPCI) inoperable following a failed Operability test. During the test, the HPCI auxiliary oil pump failed to start when the control switch was taken to start. Due to the lack of hydraulic oil pressure provided by the auxiliary oil pump, the HPCI main stop valve did not open followed by the illumination of the HPCI turbine trip annunciator, thus, preventing HPCI from starting and making it inoperable. An investigation was performed and found the secondary connection stabs on the Motor Control Center Pan Assembly feeding the auxiliary oil pump were not making adequate contact. The stab tension was adjusted and the oil pump was started followed by successful completion of the HPCI pump operability test. Because HPCI does not have a redundant system, the pump failure to start on demand represents a safety system functional failure as defined in NEI 99-02 and is reportable per 50.72(b)(3)(v)(D).

EVENT DESCRIPTION
At 0140 EST on March 19,2019, while Unit 2 was in MODE 2 (with Reactor Steam Dome Pressure> 150 PSIG) and at approximately 1 percent power, following a refueling outage, Operations declared the High Pressure Coolant Injection (HPCI) [EllS BJ] inoperable following a failed Operability test. During the test, the HPCI auxiliary oil pump failed to start when the control switch was taken to start. Due to the lack of hydraulic o~ pressure provided by the auxiliary oil pump, the HPCI main stop valve did not open followed by the iUumination of the HPCI turbine trip annunciator. The Unit entered into a required action statement (RAS) for technical specification 3.5.1 Condition C, for the auxiliary oil pump failure to start that resulted in HPCI being declared inoperable.
EVENT CAUSE ANALYSIS
The subsequent investigation and analysis determined that the secondary connection stabs, on the Motor Control Center (MCC) [EllS MCC] Pan Assembly feeding the auxiliary oil pump, were not making adequate contact in the MCC causing the auxiliary oil pump not to start. The inspection procedure failed to include the inspections of the stabs and contacts.
REPORT ABILITY AND SAFETY ASSESSMENT:
HPCI does not have a redundant system, therefore, this condition was reportable per 50.73(a)(2)(v)(D). A pump failure to start on demand represents a safety system functional failure as defined in NEI 99-02. The safety significance of this event is minimal due the a short duration of less than 14 hours1.62037e-4 days <br />0.00389 hours <br />2.314815e-5 weeks <br />5.327e-6 months <br /> that HPCI was declared inoperable and the concurrent availability of the Reactor Core Isolation Cooling [EllS BN] system. This MCC Pan Assembly only feeds the HPCI auxiliary oil pump.
CORRECTIVE ACTIONS
The stab tension was adjusted and the auxiliary oil pump started followed by the successful completion of the HPCI pump operability. The RAS was exited on March 19,2019 at 1522 EST.
Corrective action v.;u be taken to update the General Electric Low Voltage MCC Inspection procedure to include the visual inspection of the MCC subcomponents, such as the secondary connection stab tension Work orders have been generated to inspect the MCC pan assembly connection stabs on both units by end of 2020 as part of the extent of condition.
