potential risk significance and implement timely effective corrective actions Although these deficiencies were not the root or contributing causes to the actua events, they represent adverse performance which limited the licensees ability t identify and correct adverse safety conditions. Specifically, 1) station personne did not recognize the potential risk significance of the degraded A auxiliar river water pump seal and did not correct the condition in a timely manner; an 2) the safety significance assessment for a reactor protection system (RPS miscalibration event was also deficient, in that engineers incorrectly conclude that protective functions of the instrument channel were not affected Additionally, corrective actions for the RPS miscalibration event did not preclud two repeat miscalibration occurrences. |
