TS 3.3.8.1 requires that the
4.16 kV emergency bus undervoltage (degraded voltage) time delay (non-loss of coolant accident) channel be placed in trip in one hour if the allowable value is out of specification. Contrary to this on September 23, 2008, the as found time delay failed to meet the TS allowed value limit and the TS required action time was exceeded without placing the channel in trip. The inspectors evaluated the significance of this finding using
IMC 0609, Attachment 0609.04, Phase 1 Initial Screening and Characterization of Findings, and determined it to be of very low safety significance (Green) because the finding represented a design or qualification deficiency confirmed not to result in loss of operability. This was entered into Entergys corrective action program as CR-2008-03796
