This occurrence was reported to Region III, Directorate of Regulatory Operations by telephone on 2/26/82 and by telegram /mailgram on 2/26/82.

This report is submitted to you in accordance with the requirements of the Technical Specifications, Section 6.6.2.a(6) which states that personnel error or procedural inadequacy which prevents or could prevent, by itself, the fulfillment of the functional requirements of systems required to cope with accidents analyzed in the SAR require a 14 day written report.

CESCRIPTION OF EVENT During the period 4/15/81 through 5/9/81, Unit 1 RC Loon D onorated with an approx. 5% flow reduction.

The indicated flows and orotective set-points were calibrated assuming that nominal flow conditions existed.

This resulted in non-conservative, loop D, 90% flow trip setpoints and a correnponding delay in the oroduction of a reactor trip signal upon

  • As described in FSAR section 14.1.6.

The flow reduction is believed to have been caused by a S/G nozzle cover inadvertently left in the RC system during the spring 1981 refueling outage.

Pieces of hinges were dis-covered in IB and ID S/G primary hot legs on 2/24/82 and 2/25/82.

These pieces are currently assumed to be from the nozzle cover.
