Technical Specification 5.7.1 states,
High Radiation Areas accessible to personnel in which radiation levels could result in an individual receiving a
deep dose equivalent less than 1.0
rem in one hour at 30 centimeters from the radiation source or from any surface that the radiation penetrates.
Technical Specification 5.7.1, further requires in part, that each entryway to such an areashall be barricaded and conspicuously posted as a high radiation area.Contrary to the above, on October 19, 2017, a licensee system engineer identified during the performance of a maintenance and engineering inspection that a chain that functioned as the barricade for the 22
reactor coolant pump vault, a posted
high radiation area, was not installed. The licensee documented this issue in
CAP 501000004026. The inspectors determined that this issue was of very-low safety significance (Green) after reviewing
IMC 0609, Appendix C,
Occupational Radiation Safety Significance Determination Process. The inspectors determined that this finding was not an
ALARA Planning or Work Control issue; was not an
overexposure; was not a
substantial potential for overexposure; and the ability to assess dose was not compromised.
