The following is a summary of information provided by the licensee via phone and email: Two trespassers were observed by the central alarm station operator via site security cameras on the southeast end of the Maine Yankee property. Both trespassers observed to be carrying equipment to conduct manual digging for fish bait. Local law enforcement was called and responded to the Maine Yankee site. Law enforcement officers from Wiscasset Police and Lincoln County Sheriffs Department contacted both individuals and advised they had trespassed onto Maine Yankee property. Both trespassers apologized for their actions and were fully compliant with the officers. They were both advised and agreed to remain off Maine Yankee Property in the future. Their conduct was not deemed suspicious.

This is a four-hour notification, non-emergency for a notification of another government agency. This event is being reported under 10 CFR 50.72(b)(2)(xi) and 10 CFR 72.75(b)(2). At 1055 EDT on 9/10/21, an employee of a site contractor that was performing work under a contract and in possession of the immediate area where the work was being performed, was involved in a material handling accident in the owner controlled area at Three Mile Island. Londonderry Township EMS and Fire responded to render assistance to the individual. Upon arrival to the site, medical personnel declared the individual deceased. The fatality was work related and the individual was outside of the Radiological Controlled Area.

A tree has been reported to be down across the road on Injun Hollow Rd. restricting access to the site. The tree was reported to be on powerlines. The licensee has contacted Eversource for the tree removal, however, there is no estimate for removal at this time. The down tree is not currently affecting shift turnover. The licensee has not implemented contingency plans at this time. The licensee has notified the NRC Region I office (Nicholson) and the Connecticut Department of Environmental Protection.

This report is being made to provide information to the NRC regarding Monticello Dry Shielded Canister (DSC)-16. On February 17, 2014, dye penetrant examinations were performed on the outer top cover plate (OTCP) to shell weld on dry shielded canister (DSC)-16. This was a re-examination of a linear indication identified on January 24, 2014. The results of the re-examination identified a 1.6 inch linear indication that remained after surface conditioning. This indication had not been previously detected by nonconforming nondestructive examination previously reported by TriVis Inc. Xcel Energy is evaluating the condition and will remedy prior to moving the cask to the ISFSI (Independent Spent Fuel Storage Installation) pad. The associated DSCs loaded during the current campaign successfully passed their helium leak tests. Helium leak checks are performed to demonstrate confinement and boundary integrity. Thus, public health and safety is not affected. Since the licensee communication plan also notified other government agencies, this report is being made pursuant to 10CFR50.72(b)(2)(xi) and 72.75(b)(2). The above referenced dry shielded canister is currently located on the refuel floor in the reactor building. The licensee notified the NRC Resident Inspector.

A sewage line has been found broken and leaking. The area of the leak is on Maine Yankee property but outside the licensed area. Repairs are scheduled for 04/09/13. No radioactive material is involved.
