The following information was provided by the licensee via email: On 12/8/2022, Prairie Island Nuclear Generating Plant initiated a notification to the State of Minnesota due to a HVAC coolant leak reaching waters of the state. The estimated quantity is 5 gallons of NALCO LCS-60. The leak was due to a failed heat exchanger coil and has been isolated. This notification is being made solely as a four-hour, non-emergency notification for a Notification of Other Government Agency. This event is reportable in accordance with 10 CFR 50.72(b)(2)(xi). There was no impact on the health and safety of the public or plant personnel. The NRC Resident Inspector has been notified.

The following information was provided by the licensee via email: At 0019 EST on 12/9/2022, the Prairie Island Nuclear Generating Plant (PINGP) made Event Notification 56266 notifying the NRC of an environmental report to the State of Minnesota due to an estimated 5 gallons of NALCO LCS-60 that leaked from a failed heat exchanger coil and reached the waters of the state. This event notification was made in accordance with 10 CFR 50.72(b)(2)(xi). During further review of NRC reporting guidance, PINGP has concluded that the reported quantity of NALCO LCS-60 that leaked during this event was below the reporting threshold outlined in NUREG 1022, Revision 3. ENS 5537723 July 2021 15:40:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Fish KillAt approximately 1040 CDT, July 23, 2021, the Minnesota State Duty Officer was notified by Xcel Energy Environmental Services of a fish kill in the Prairie Island Nuclear Generating Plant discharge canal. The fish kill resulted from a change in temperature due to the loss of power to the plant cooling tower pumps. The cause of the power loss is under investigation. This notification is being made as a four-hour, non-emergency notification in accordance with 10 CFR 50.72(b)(2)(xi). There was no impact on the health and safety of the public or plant personnel. The NRC Resident Inspector has been notified.
ENS 548773 September 2020 19:21:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to a Hydraulic Oil SpillOn 9/3/2020, at 1421 (CDT), a notification was made to the National Response Center as a result of a spill of approximately 3 fluid ounces of hydraulic oil into the plant waters of Prairie Island Nuclear Plant intake on 9/2/2020. This notification is being made as a four-hour, non-emergency notification for a Notification of Other Government Agency. This event is reportable in accordance with 10 CFR 50.72(b)(2)(xi). There was no impact to public health and safety. The NRC resident inspector has been notified.
ENS 5375728 November 2018 06:00:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Inadvertent Siren ActivationAt 0752 CST, on November 28, 2018, Dakota County inadvertently actuated their sirens while performing a scheduled weekly (Emergency Planning Fixed Siren Test). All seven (7) Dakota County sirens actuated for approximately 9 seconds before Dakota County Dispatch canceled the activation. This 4-hour non-emergency report is being made per 10 CFR 50.72(b)(2)(xi), Offsite Notification (which was made to Dakota County Dispatch). Capability to notify the public was never degraded during this time. All Emergency Notification sirens remain in service. No press release is planned at this time. The licensee has notified the NRC Resident Inspector.
ENS 5274210 May 2017 12:55:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Inadvertent Siren ActuationAt approximately 0755 CDT, on May 10, 2017, Pierce County inadvertently actuated their sirens while performing a scheduled weekly cancel test. All fifty two (52) Pierce County sirens actuated county wide for approximately 11 seconds before Pierce County Dispatch canceled the activation. This 4-hour non-emergency report is being made per 10 CFR 50.72(b)(2)(xi), Offsite Notification. Capability to notify the public was never degraded during this time. All Emergency Notification sirens remain in service. No press release is planned at this time. The license has notified the NRC Senior Resident Inspector.
ENS 5149724 October 2015 11:57:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification for Onsite Incident of Local InterestPrairie Island Nuclear Generating Plant (PINGP) notified Local Law Enforcement Agency and the Prairie Island Community Tribal Council due to an incident onsite of local interest. This notification is being conducted in accordance with 10 CFR 50.72(b)(2)(xi) for notification to an outside government agency. The plant remains safe, and this condition does not pose any additional risk to the public. The licensee has notified the NRC Resident Inspector.
ENS 509667 April 2015 22:45:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseInadvertent Emergency Siren ActivationAt approximately 1833 CDT on April 07, 2015, the licensee was notified by the siren vendor (NELCOM) that Dakota County Dispatch reported an inadvertent activation of an emergency siren (D-1), in Dakota County, MN. The cause of the siren activation is unknown. The siren was deactivated at 1810 CDT after sounding for approximately 25 minutes. The siren vendor (NELCOM) has been contacted to repair the siren. The siren remains out of service. This is the only siren out of service within the 10 mile Emergency Planning Zone (EPZ). There are 123 Emergency Notification sirens. Of the seven (7) credited sirens in Dakota County, six (6) remain in service. Thus, emergency notification capabilities remain in effect. NRC Resident has been informed.
ENS 505116 October 2014 15:59:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Fish Found Dead After Zebra Mussel TreatmentAt 1059 CDT on October 6, 2014, the State of Minnesota and Environmental Services were notified that approximately 100 fish were found dead against the trash racks of the plant screen house. The licensee had completed zebra mussel treatment several days prior to this event and the fish deaths are attributed to this treatment. This is reportable under 10 CFR 50.72 (b)(2)(xi), News Release or Notification of Other Government Agency. The protection of the health and safety of the public was not affected by this issue. The licensee has notified the NRC Senior Resident Inspector.
ENS 5035812 August 2014 17:48:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Inadvertent Siren ActuationAt approximately 1248 CDT, on August 12, 2014, Emergency Planning was notified by the siren vendor that Pierce County inadvertently actuated sirens at 1239 (CDT) while performing a cancel test. Fifty two (52) of the 123 sirens were actuated county wide for approximately 15 seconds before Pierce County could cancel activation. Per F3-5.2, Response to a False Siren Activation, this requires a 10 CFR 50.72(b)(2) 4 hour non-emergency report. Capability to notify the public was never degraded during this time. The sirens remain in service. No press release is planned at this time. The license has notified the NRC Senior Resident Inspector.
ENS 5023628 June 2014 17:15:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification to the State of Minnesota for a Fish KillAt 1215 PM CDT on June 28, 2014, Xcel Energy notified the Minnesota State Duty Officer that approximately 1000 dead fish of assorted species were discovered in the site's discharge canal. The fish perished due to rising temperatures in the discharge canal caused by an orderly shutdown of Cooling Towers as directed by procedure for response to flooding of the Mississippi River. The discharge canal is a body of water with a barrier from the Mississippi River. The Mississippi River temperature across the site has only risen 0.9F which is well within the permit limit of the maximum of 5F. There is no impact to the availability of safety systems or safety system performance. This condition has no impact to public health and safety. This event is reportable per 10 CFR 50.72(b)(2)(xi), an event related to protection of the environment for which a notification to other government agencies has been made. The licensee has notified the NRC Resident Inspector (and local authorities).
ENS 5020316 June 2014 15:27:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Hydraulic Oil SpillAt approximately 1027 CDT on June 16, 2014, Xcel Energy notified the Minnesota State Duty Officers that a hydraulic hose on a cement truck failed and spilled approximately 15 gallons hydraulic oil to grade at a building site for the new Site Administrative Building. The hydraulic oil spill did not enter the storm drain system or any water ways. The site is in the process of cleaning up the spill. There is no impact to public health and safety or the environment due to this incident. This event is reportable per 10 CFR 50.72(b)(2)(xi), an event related to protection of the environment for which a notification to other government agencies has been made. The licensee has notified the NRC Resident Inspector.
ENS 4977830 January 2014 19:20:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Inadvertent Activation of One Emergency Siren

One siren false actuation. At approximately 1415 CST on January 30, 2014, the licensee was notified of a false activation of emergency siren (G-14). The site contacted the siren vendor. It was determined that the siren had falsely actuated 5 to 6 times and has since been deactivated. The Goodhue County Sheriff's department received calls from some area residents. The siren remains out of service and is the only siren out of service within the 10 mile Emergency Planning Zone (EPZ). NRC Resident Inspector has been informed.

Based upon further review, the event time of the false siren actuation was approximately 1320 CST. The previously reported event time of 1415 CST was the confirmation time of the siren vendor responding to the siren actuation. This does not represent a significant loss in emergency notification. The siren system has significant overlap between sirens. For any failed siren, backup methods are implemented per our emergency plan to ensure timely public notification. The licensee notified the NRC Resident Inspector of this update. Notified R3DO (Sheldon).

10 CFR 50.72(b)(3)(xii), Transport of a Contaminated Person Offsite
Transport of Potentially Contaminated Worker OffsiteOn 09/22/2013 at 1940 CDT, Prairie Island Nuclear Generating Plant (PINGP) requested an offsite ambulance via the 911 system for medical assistance for an individual in the radiologically controlled area. The person was treated as potentially contaminated because a complete survey to confirm the absence of contamination was not completed prior to transport of the individual. An ambulance arrived on site at 1956 CDT and departed the site at 2036 CDT to the Red Wing Minnesota Hospital. PINGP radiation protection personnel accompanied the individual to the hospital. A survey at the hospital determined that the individual was not contaminated. This is considered a transport of a potentially contaminated individual requiring an 8 hour ENS Notification per 10CFR50.72(b)(3)(xii). The Prairie Island Indian Community was notified of the transport of an individual by ambulance. This is considered a notification of another government agency and an event that may have potential interest to the media requiring a 4 hour ENS Notification per 10CFR50.72(b)(2)(xi). The licensee notified the NRC Resident Inspector.
ENS 4892515 April 2013 15:18:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to an Inadvertent Activation of a SirenAt approximately 1018 CDT on April 15, 2013, the licensee was notified that the Pierce County Sheriff Dispatch reported an inadvertent activation of an emergency siren (P-43), in Pierce County, WI. The cause of the siren activation is unknown. The siren was deactivated at 1022 CDT after sounding for approximately 28 minutes. The siren vendor (NELCOM) has been contacted to repair the siren. The siren remains out of service and is the only siren out of service within the 10 mile Emergency Planning Zone (EPZ). NRC Resident has been informed (by the licensee).
ENS 487113 February 2013 18:08:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Release of Amertap BallsAt 1208 CST, Xcel Energy notified the Minnesota State Duty Officer that up to 950 Amertap Balls had been lost from the Prairie Island Nuclear Generating Plant Unit 2 Condenser Tube Cleaning System. Since the Minnesota State Duty Officer for the Division of Emergency Management was notified, this constitutes a 4 hour non-emergency notification per 10CFR50.72(b)(2)(xi). The licensee notified the NRC Resident Inspector. The licensee also notified the local county sheriff's office.

ENS 4840112 October 2012 02:07:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Onsite Sirens Alarming During a DrillOn 10/11/12 the Prairie Island Nuclear Generating Plant (PINGP) Security Force was conducting scheduled force on force security drills from 2000 to 2330 CDT. At 2107 CDT the Licensee was notified by the Wisconsin State Radiological Coordinator (SRC) that an intern had contacted the Wisconsin State Patrol Duty Officer to say they had taken cover at the plant and that there were sirens going off in the background. The intern did state that he did not know whether this was a real event or a drill. The Wisconsin State Patrol contacted the office of Wisconsin Emergency Management and the SRC. The SRC then contacted the PINGP Control Room and was informed by control room personnel that this was a security force-on-force drill. Xcel Energy Communications has notified other government agencies of this event via an email. No press release is planned at this time. This event is reportable per 10 CFR 50.72(b)(2)(xi). The licensee has notified the NRC Resident Inspector.

Since the Minnesota State Duty Officer was contacted, this constitutes a 4 hour non-emergency notification per 10CFR50.72(b)(2)(xi). The NRC Resident Inspector has been notified.

Since the Minnesota State Duty Officer was contacted, this constitutes an 4 hour non-emergency notification per 10 CFR 50.72(b)(2)(xi). The licensee has notified the NRC Resident Inspector. The licensee will notify local and other government agencies.

Since the Minnesota State Duty Officer was contacted, this constitutes an 4 hour non-emergency notification per 10 CFR 50.72(b)(2)(xi). The licensee has notified the NRC Resident Inspector in addition to state and local government agencies.
ENS 476406 February 2012 19:00:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Unpermitted ReleaseThe Minnesota Pollution Control Agency (MPCA) was notified of a release of 27 gallons from the condensate system. The release contained the following chemicals: 1) 11.2 ppm of methoxy propylamine (MPA); 2) 8.1ppm of ammonia; 3) 11.2 ppb of hydrazine; 4) 1.50 E-5 microCi/mL (15,000 pCi/L) of tritium. The condensate was released from parts Warehouse #1 heating steam system overflow vent. Condensate return pumps failed to operate causing 27 gallons of condensate to backup and overflow onto the ground. A catch basin has been installed and the steam inlet valve has been closed preventing the addition of steam into the system. The NRC Resident Inspector has been informed. The licensee also notified the Prairie Island Dakota Indian Community.
ENS 4762930 November 2011 13:58:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Unmonitored Release of Tritium (Heating Steam Condensate)Heating Steam condensate containing 9,430 picocuries per liter (pCi/L) of tritium overflowed from the Main Warehouse between November 22, 2011 and November 29, 2011. The US EPA drinking water limit for tritium is 20,000 pCi/L. The Warehouse Condensate Return system failed to operate which resulted in the overflow of the Heating Steam condensate. There was a potential for the condensate to enter the environment where it could reach groundwater. The released volume for this event was conservatively calculated to be up to 3,900 gallons. The 122 Condensate Return Pump was replaced and the system was restored to service. This voluntary report is in accordance with the Nuclear Energy Institute's Industry Ground Water Protection Initiative. The licensee has notified the NRC Resident Inspector and will inform state and local agencies of this event.
ENS 4710027 July 2011 14:04:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification of Siren Activation by Lightning StrikeAt approximately 0904 CDT on July 27, 2011, the licensee was notified of an inadvertent activation of emergency siren (G-03) in the township of Welch, MN. The site contacted the siren vendor. It was determined that the siren was damaged by a lightning strike and has since been deactivated after sounding for approximately 35 minutes. The Goodhue County Sheriff's department received calls from some area residents. The siren remains out of service and is the only siren out of service within the 10 mile Emergency Planning Zone (EPZ). The licensee stated that the vendor is working on repair of the siren. The licensee has notified local authorities and the NRC Resident has been informed.
ENS 4709124 July 2011 00:00:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Unmonitored Release of Tritium to Septic System

It was discovered that water containing tritium from a secondary system steam leak has been periodically disposed of via the septic system versus the normally monitored turbine building sump effluent path. The tritium amount was 8.00 E-5 microcuries per milliliter. The tritium sample is consistent with normal secondary plant samples. Since November, 2010, there have been no abnormal tritium results from the septic system. There was also MPA (29.7 parts per million) which is a pH additive. The septic system is monitored on a monthly basis but is not a normal path for discharge of equipment related fluid leakage. The steam has been condensed and collected in a 5 gallon pail that is periodically emptied by crew personnel. Management just became aware that there were instances where disposal of the water was to the septic system. This practice has been halted. The steam leak has been active since November of 2010. This is being reported due to the potential for being an unusual or abnormal release of radioactive effluent. The NRC resident has been informed. The licensee will also inform state and local agencies of this event.

This is an update to EN 47091 (Water containing tritium from secondary system leak disposed of via septic system) submitted at 2320 EDT on 7/23/2011. The fluid in the septic system at Prairie Island Nuclear Generating Plant is removed from site by a vendor, who transports the fluid to the Red Wing, MN Wastewater Treatment Plant where it is processed prior to being discharged. The NRC Resident Inspector has been informed. Notified R3DO (Passehl).

ENS 4704111 July 2011 06:25:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseInadvertent Emergency Siren Activation Coincident with Adverse WeatherAt approximately 0125 CDT on July 11, 2011 licensee was notified by Goodhue County Sheriff's Department of an inadvertent emergency siren activation (R17) in the city of Red Wing, MN. The Sheriff's Department deactivated the siren and notified the siren vendor. Siren initiation was coincident with severe storm activity in the area, including lighting strikes. Siren remains out of service and is the only siren out of service within the 10 mile Emergency Planning Zone (EPZ). The licensee informed the NRC Resident Inspector.
ENS 464648 December 2010 14:07:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Inadvertent Actuation of Emergency SirensAt approximately 0807 CST on December 8, 2010, Pierce County (Wisconsin) Dispatch inadvertently activated the Pierce County sirens during the conduct of the silent siren test. The sirens were deactivated within 30 seconds. The licensee has notified the NRC Resident Inspector.
ENS 4617615 August 2010 15:45:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Discharge of Amertap Neoprene Balls Into WaterwayDuring the performance of TP-1539, Unit 1 Amertap ball check on 8/15/10 at 0933, it was discovered that approximately 1300 balls could have been lost to the environment via the external circulating water system from the #12 Amertap Ball System. The Xcel Energy environmental service informed the plant that reports were made to the Minnesota Pollution Control Agency at 1050 CDT on 8/15/10 and the Minnesota State Duty Officer at 1045 CDT on 8/15/10. The Prairie Island Indian Community Tribal Council will be notified. The #12 Amertap Ball System is shutdown and will remain out of service until repairs are completed. The licensee informed the NRC Resident Inspector.Circulating Water System
ENS 461629 August 2010 13:12:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Loss of Condenser Tube Cleaning (Amertap) BallsDuring the performance of TP-1539, Amertap Ball Check on 8/08/2010 at 1326 (CDT), it was discovered that (about) 1400 balls could have been lost to the environment (Mississippi River) via the External Circulating Water System from the 11 Amertap Ball System. The Xcel Energy Environmental Service informed the plant that reports were made to the Minnesota Pollution Control Agency on 8/09/2010 at 0817 and the State of Minnesota Duty Officer on 8/09/2010 at 0812. The Prairie Island Indian Community Tribal Council was notified. The 11 Amertap Ball System is shutdown and will remain out of service until repairs are completed. ENS 461619 August 2010 12:42:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseDiscovery of Firearm During X-Ray ScreeningAt 0742 CDT on 08/09/2010, Prairie Island Nuclear Generating Plant (PINGP) personnel discovered a hand gun in a personal bag being processed through an x-ray search machine in the Security Access Facility. Security personnel immediately established control of the weapon and the owner. The owner of the weapon is a permanent employee of PINGP. The owner has been determined to hold proper permits for the weapon. The Local Law Enforcement Agency and the Prairie Island Community Tribal Council was notified. The NRC Resident Inspector has been notified. This notification is being conducted in accordance with 10CFR50.72(b)(2)(xi) for notification to an outside government agency.
ENS 4600413 June 2010 05:35:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Inadvertent Siren ActivationNotification of an inadvertent siren activation by Goodhue County (MN) authorities. At 0035 (CDT) on June 13, 2010, the Prairie Island Shift Manager was notified by plant security that the Goodhue County Dispatcher had inadvertently pressed a touch screen and activated all sirens in the county. The sirens were deactivated within 10 to 15 seconds. The (NRC) Resident Inspector has been informed (by the licensee).
ENS 4596631 May 2010 02:15:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseLate Notification of a Single Inadvertent Siren Activation During Thunderstorm ActivityAt approximately 2115 CDT on May 30, 2010, a Pierce County dispatcher was notified of inadvertent siren activations, momentarily activating for approximately 30 minutes. The dispatcher also indicated there were lightning strikes in the area about the same time. The siren vendor was dispatched and the issue was tracked down to a single siren; no physical damage was observed, however, the siren was declared out of service by the siren vendor. The licensee notified the NRC Resident Inspector.

This resulted in a loss of amertap balls to the external circulating water system. The quantity of balls lost to the environment is unknown but could potentially be the entire quantity of 1500 balls. The Xcel Energy Environmental Service informed the plant that a report will be made to the Minnesota Pollution Control Agency during business hours on 4/19/2010. The NRC Resident Inspector has been informed. A news release is not planned. The Amertap system is out of service until repaired. The licensee will also be notifying the Prairie Island Indian Nation and Goodhue County, MN of this incident.Circulating Water System
ENS 4584114 April 2010 21:20:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notifications Due to Fish Kill and Chemical ReleaseThe licensee notified the State of Minnesota Department of Natural Resources and the Minnesota Pollution Control Agency (MPCA) at 1620 CDT of a fish kill that occurred during a change to the plant circulating water system. Approximately 300-400 fish were found dead in the discharge canal during inspections related to a Zebra Mussel Treatment in progress. The fish included carp, green sunfish, bluegill, carpsuckers, and gizzard shad. The cause appears to be related to temperature changes in the canal as the external circulating water system was manipulated to achieve the 82-84 deg (F) condenser inlet temperatures needed for the treatment and a reduction in plant blowdown flow to the river required by the site discharge permit. The dead fish were contained in discharge canal due to a net that was deployed on Monday in preparation for zebra mussel treatment conducted today (treatment started at 1030 am). Fish in the discharge canal have been dead for 12 to 24 hours. The fish kill is unrelated to the zebra mussel treatment mulluscide addition and did not extend to state waterways. During the Zebra Mussel treatment, detectable mulluscide was found at the sluice gates to the Mississippi River at 1607 and the chemical addition was stopped The chemical is normally neutralized by addition of clay prior to reaching the sluice gates. The 1607 and 1620 samples at the sluice gates showed 0.6 ppm and 0.8 ppm of CL-2005 mulluscide and the 1730 sample had returned to non-detectable. The plant notified the Minnesota Department of Emergency Management of the chemical release at 1856, event number 110570. The MPCA will be notified during business hours tomorrow. ENS 4573428 February 2010 07:45:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification of State and Tribal AgenciesAt 0145 Prairie Island Security personnel detected an unknown person within the Owner Controlled Area. The individual was walking from the area near the Mississippi River boat ramp West and was intercepted by Security personnel in the plant parking lot. He was then taken into custody by Prairie Island Police. The event was determined to be media sensitive and courtesy notifications are being made to state, local and tribal agencies. No news release is planned and there was no effect on plant operations. It was later determined the individual had fled from the Treasure Island Casino. Local police had been searching for him at the time plant Security stopped him." The individual had been detained at the casino due to suspected underage drinking and was being held for local law enforcement when he fled on foot. The licensee notified the NRC Resident inspector.
ENS 4564120 January 2010 19:17:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Firefighting Assistance for Fire in a Non-Safeguards Diesel GeneratorWhile testing, a fire occurred in the D3 Non-Safeguards Diesel Generator at 1317 (CST). CO2 was applied and at 1337 (CST), the fire was extinguished. A re-flash occurred at 1413 (CST) requiring further fire brigade and fire department action and was extinguished at 1455 (CST). No injuries resulted from this fire and it was contained to the D3 Building. The D3 Building is not adjacent or contiguous to any vital areas, and therefore, entry into Emergency Action Levels was not required. This fire did not pose a threat to the safe operation of the plant. The Red Wing Fire Department was called and arrived on site to assist the plant fire brigade. This resulted in declaring a media sensitive event requiring government agency notifications, and therefore, the need for this report. The investigation of the cause and extent of damage is ongoing. The (NRC) Resident Inspector has been informed.

OFFSITE NOTIFICATION RESULTING FROM AN INADVERTANT ACTIVATION OF A SIREN

Prairie Island Nuclear Plant was notified by Goodhue County Dispatch that a public alert notification siren inadvertently activated at 0430 hours. Local law enforcement officials deactivated the siren at 0500 hours. (Greater than) 70% of sirens remain functional in the 10 mile EPZ (Emergency Planning Zone). The communications company has been contacted to investigate the cause of the false activation.

The NRC Resident Inspector has been notified.

ENS 4546624 October 2009 18:20:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification of Minor Oil SpillA light film of oil was discovered on the recycle canal of the plant water ways. The source was from residual oil on the deck of a small barge parked on the canal. Snow melt-off carried the oil into the canal. The barge is being used to dredge the canal. The quantity was estimated at less than 1/2 cup (of oil). The oil is being cleaned up with oil absorbent booms and pads. The oil was contained to the plant water ways and was not discharged to the Mississippi River. Xcel Energy Services was notified. Xcel notified the National Response Center (Rpt #921543) and the Minnesota Division of Emergency Management (Rpt #107021). The licensee has covered the barge to prevent any further spill and will continue to monitor conditions. The NRC Resident has been notified.
ENS 4543715 October 2009 19:49:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Minor Oil Spill to the Mississippi RiverAn oil spill of less than 1/2 of a cup to the Mississippi River occurred while preparing for Intake Structure dredging activities. Hydraulic oil leaked out of a fitting for the dredging pump and entered the Mississippi River side of the Intake Screenhouse. A small oil sheen was noticed and collected using the pre-deployed absorbents. The fitting was repaired and no more oil is currently leaking. The Minnesota Pollution Control Agency and the National Response Center were notified. The licensee notified the NRC Resident Inspector.
ENS 454062 October 2009 16:49:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Inadvertent Activation of Emergency SirensAt approximately 1149 CDT, on October 2, 2009, a Goodhue County Dispatch Supervisor inadvertently activated the Red Wing (Minnesota) sirens from the backup unit during conduct of a silent siren test. The backup unit was being tested to ensure proper siren function prior to work on the primary unit. Twenty-six of the 117 sirens in the 10-mile Emergency Planning Zone (EPZ) were activated for less than 10 seconds. No press release is planned by Goodhue county or Xcel Energy. The NRC Resident Inspector was notified.
ENS 4535717 September 2009 16:30:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification to Minnesota Pollution Control AgencyApproximately one cup of hydraulic oil entered the plant waterway while operating the 122 Emergency Intake Bypass Gate. While troubleshooting a limit switch issue with the bypass gate, a leak developed on the lower seal for the hydraulic cylinder. An oil sheen was noticed on the plant side of the Intake Screen House and has been cleaned up with absorbents. No oil sheen was detected on the Mississippi River side of the Intake Screen House or in the plant discharge canal. The Minnesota Pollution Control Agency and the National Response Center were notified. The licensee has notified the NRC Resident Inspector. Licensee will be contacting local and other government agencies.

ENS 4512913 June 2009 18:53:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Emergency Siren ActuationAt approximately 1353 (CDT), (the licensee) received notification of a single siren actuation in the Early Warning Siren System. This call came from a Pierce County 911 dispatcher after they received a call from a local resident. The siren system vendor reports the siren actuated at approximately 1310 and was inactivated by Pierce County Deputies at 1345 using the local emergency key. The siren (P14) remains out of service and is the only siren in Pierce County currently out of service. The licensee's procedural requirement to maintain at least 70% of sirens within the 10 mile EPZ is still met. Neither Pierce County nor the licensee intend on making a press release. The licensee notified the Pierce County Emergency Government Manager. The licensee notified the NRC Resident Inspector

On June 9, 2009 at approximately 1200 CDT, remains not yet identified as human or animal were found by Prairie Island Nuclear Generating Plant (PINGP) employees. The remains were discovered on Xcel Energy property just outside the PINGP owner controlled area along the transmission line path, near the railroad tracks. The Red Wing City Police Department and the Prairie Island Indian Community Police Department have both been notified and are on the scene. Notifications are being made to appropriate agencies and officials. No press release is planned." The licensee has notified the NRC Resident Inspector. The licensee is treating this as a media sensitive event.

ENS 4494731 March 2009 17:00:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseEmergency Siren Activated for Reasons UnknownOn 3/31/2009 at 1323 CDT Prairie Island Nuclear Generating Plant was notified by the site's siren contractor (NELCOM) that Pierce County siren P-20 had falsely activated at around 1200 CDT. Pierce County notified NELCOM of the siren activation after receiving several calls from the public. No press release is planned for this event. Siren P-20 (1 of 52 sirens) has been removed from service and backup route alerting will be used in the event siren activation is required. The cause of the false activation is being investigated and will be repaired. The NRC Resident Inspector has been notified. At 1544 CDT siren P-20 was repaired and returned to service.

Twenty-six (26) of the 117 sirens in the 10 mile Emergency Planning Zone (EPZ) were activated for approximately two minutes. No press release is planned. The licensee notified the NRC Resident Inspector.

The driver of the vehicle was not a plant employee. The driver left the scene of the accident. Plant security and local law enforcement are investigating. No press release is planned. The licensee notified the NRC Resident Inspector, the Emergency Management agencies of Goodhue, Dakota, and Pierce counties, the State of Wisconsin and the Prairie Island Tribal council.

ENS 4437028 July 2008 13:50:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to a Sodium Hypochlorite SpillAt approximately 0850 (CDT) on July 28, 2008 Xcel Energy notified the State of Minnesota's Duty Officer of a 53 gallon 12% Sodium Hypochlorite spill at the Prairie Island plant. The spill was due to a leak in the chemical piping. The release was stopped but only partially contained within the berms of the building. Some of the Sodium Hypochlorite seeped through the berms concrete walls and into the environment. The amount escaping the containment berm was less than the reportable quantity but was being reported since a portion did reach the soil. The licensee informed both the State and the NRC Resident Inspector.

ENS 4409526 March 2008 12:58:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Inadvertent Actuation of Emergency SirensAt 0758 CDT, on March 26, 2008, the Goodhue County dispatcher inadvertently activated the Goodhue County sirens from the primary unit during the conduct of a silent siren test. As per procedure they were to send out two silent signals; however the first was the silent and the second signal was the activation. Upon selecting the alert, they immediately sent out a signal to cancel the activation. Thirty-two of the 117 sirens in the 10-mile Emergency Planning Zone (EPZ) were activated for approximately 14 seconds. The licensee was notified of the siren activation by the activation signal received on the informer unit in the Technical Support Center (TSC) as well as hearing the sirens being activated. Goodhue County Emergency Management contacted the local radio station at 0805 (hrs. CDT) to notify the public of the false activation. No press release is planned by Xcel Energy. No other press release is planned by Goodhue County. The licensee notified the NRC Resident Inspector.
