ATTACHMENTTOAEP:NRC:1212Page22.Procedure12IHP6030.IMP.030,Re~ision5,wasnotdevelopedinaccordancewithPMI-2010inthattheprocedurewasnotdesignatedIn-Handassignifiedbydoubleasterisksprecedingtheprocedurenumber.ThisisaSeverityLevelIVviolation(SupplementI)."ResponsetoViolationl.AdmissionorDenialoftheAlleedViolationIndianaMichiganPowerCompanyadmitstotheviolationascitedintheNRCnoticeofviolation.2.ReasonsfortheViolatioOnFebruary21,1994,correctivemaintenancetorepairleakbywasperformedontheUnit2mainsteamstopvalvedumpvalve,2-MRV-241.Aspartofthismaintenanceactivity,valve2-MRV-241wastobesetupandstroked.Thesetupandstrokesetwasperformedwithoutanin-handprocedure.PlantManager'sInstruction,PMI-5020,"CorrectiveMaintenance"identifiestheneedtopreplanworkonsafety-relatedequipmentandtoperformtheworkinaccordancewithwrittenprocedures,documentedinstructions,ordrawingsappropriatetothecircumstances.Uponreviewoftheabovemaintenanceactivity,ithasbeenconcludedthattheworkinvolvedinsettingthecorrectstrokeforthisvalveistoocomplextobeperformedwithoutanin-handprocedure.Wefailedtoidentifythatinadequateinstructionexistedattheworksitetoperformthetaskinquestion.3.CorrectiveActionsTakenandResultsAchievedPriortounitstartup,workactivitieswereperformedtocorrectthepre-loadvaluesofthevalve.Thebenchset,withtheactuatoruncoupledfromthevalve,wasverifiedtobecorrectfortherequiredstrokelength.Theactuatorwasthenproperlycoupledtothevalvetoachievetherequiredstrokelengthandpressurevalues.Testingwasperformedtoverifyproperoperation,requiredstrokelength,andrequiredpressurevalues.Procedure12IHP6030.IMP.030wasenhancedanddesignatedasanin-handprocedure.
ATTACHMENTTOAEP:NRC:1212Page34.CorrectiveActionsTakentoAvoidFurtherViolationsMaintenancewillreviewthestandardsapplicabletoqualitywork,andre-evaluatetheirprocedurescurrentlydesignatedasnon-in-handagainstthesestandards.Thereare30oftheseprocedures.These30procedureswillberevised,enhancedtocontainappropriatelevelsofdetail,andredesignatedasin-handproceduresorcanceled,asappropriate.Themaintenancescreeningprocesschecklist,MA3.3-05,Attachment5,wasrevisedtoclarifyrequirementsforproceduresattheworksite.Lessonslearnedfromthiseventweresharedwithallotherdepartmentswhichperformmaintenancework.5.DateWhe11ComlianceVilBeAchievedMaintenanceadministrative.processMA3.3-05,Attachment5wasrevisedApril27,1994.The"LessonsLearned"memowasissuedMay6,1994.ProcedurerevisionsandredesignationwillbecompletedbyJuly1,1994.FullcompliancewasachievedonApril6,1994,whenprocedure12IHP6030.IMP.030wasrevisedandre-issuedasanin-handprocedure.
