TS 3.7.C,
Secondary Containment, requires, in part, that whenever the reactor is critical,
secondary containment integrity must be maintained. Contrary to this, on January 10, 2008,
damper AO-N-78, the Reactor Building Isolation Control System ventilation
damper, failed to go fully closed on demand during testing, resulting in
secondary containment integrity not being maintained. TS actions to restore operability or to secure the
damper were not taken for approximately
96 hours0.00111 days <br />0.0267 hours <br />1.587302e-4 weeks <br />3.6528e-5 months <br />. This was identified in Entergys
CAP as
CR-PNP-2008-140 and
CR-PNP-2008-143. This violation is of very low safety significance (Green) because it did not represent an open pathway in
secondary containment due to the availability of a backup isolation
