FinalDesignDeficiencyReportTargetRockValves~SummarAdesigndeficiencywasidentifiedinvalvesmanufacturedbyTargetRockCorporationandfurnishedbyCombustionEngineering.Duetothedesigndeficiencythevalves,whenoperatedunderdesignconditions,wouldbepronetofailure.Thefailure.wouldconsistofthediscseparatingfromthestem,cracksinthevalveseatandbackseat,andscoringofthediscbelowtheseatingsurface.Inaccordancewiththerequirementsof10CFR50.55(e),theabovedeficiencyisconsideredreportable.BytelephoneFPLnotifiedtheNRConNovember24,1980,andaninterimreportwassubmittedonDecember23,1980(FPLLetterL-80-414).ThisfinalreportissubmittedtoadvisetheNRCofthedescrip-tionofthedeficiencyandthecorrectiveactionsthatwillbetaken.DescritionOurNSSSvendor(CombustionEngineering)andMr.C.R.McFarland(NRC)informedFPLthata.designdeficiencyhadbeenidentifiedinTargetRockvalves(8"motor-operatedglobevalve)attheSanOnofreNuclearGeneratingStation(SONGS)andhadbeenreportedbySouthernCaliforniaEdisontotheNRCunder10CFR50.55(e).AninvestigationconcludedthatSt.LucieUnit2,globevalvesHCV3615,HCV3625,HCV3635,HCV3645,V3536,V3539,V3550,andV3551performthesameflowthrottlingserviceasthevalvesintheSONGSunitandaresimilarindesigntotheSONGSvalvesthatfailed,exceptthatthe,St..Lucievalvesare6"(HCV3615,HCV3625,HCV3635,HCV3645),4"(V3536,V3539)and'"'V3550,V3551)insize.Thevalvefailure.at,SONGShasbeenattributedtoexcessflowinducedvibrationwhilethevalvediscwasinapartiallyopenposition.Cracksinthevalveseatandbackseatweld.a'ndscoringofthevalvediscbelow.theseatingsurfacewouldresultfromimpactofthevibratingdiscontheseat.CorrectiveActionTargetRockCorporationhasmodifiedtheirvalvedesigntoeliminateflowinducedvibrationbyprovidingadiscwhichisguidedbytheseatinthepartiallyopenedposition.,Allaffected6",4",and3"valvescitedabovewillbereturnedtothevalvemanufacturerforinspection,rework,modifica-tionandtesting;SafetImlicationThisdeficiencycouldhaveadverselyaffectedthesafetyofoperationsatthe.plantduringits40yearlifeifitweretohaveremaineduncorrected.Itrepresentedasignificantdeviationfromperformancespecificationswhichrequiredrepairstoestablishtheadequacyofthecomponenttoperformits intendedsafetyfunction.Theaffectedval,vesarepartoftheSafetyInjectionSystemwhosesafetyrelatedfunctionisto-insurethatadequateflowisdeliveredtotheReactorCoolantSystem,thusinsuringadequatecorecooling.Withouttheassuranceofvalvecomponentintegritywhensubjectedtothedesignbasisserviceconditions,safetyofplantoperationscouldbejeopardized.V.ConclusionCorrectiveactionasindicatedinSectionIIIofthisreportwillbeunder-,takenbythe,vendortoassure.thatthevalvesareabletoperformtheirintendedsafetyfunction.
