On August 12, 2025, the licensee was conducting radiography with a Sentinal 880 Delta (serial number 4955) containing a 73.9 Ci Ir-192 source when it was discovered that the source was stuck in the unshielded position. The radiography crew called the radiation safety officer (RSO) and ensured the cordoned off area was clear of personnel. With assistance from the RSO, the crew was able to retract the source to the shielded position approximately 3.75 hours8.680556e-4 days <br />0.0208 hours <br />1.240079e-4 weeks <br />2.85375e-5 months <br /> later.
TLD badges for personnel present were sent for expedited analysis, and direct reading dosimeter readings are listed below:
