The inspection was an examination of the activities conducted under your license as they relate to radiation safety and to compliance with the Nuclear Regulatory Commission (NRC) rules and regulations and the conditions of your license. The inspection consisted of selective examinations of procedures and representative records, interviews with personnel, and observations by the inspector. 1165 S. Linden Road, Flint, MI 48532 1 Scope and Observations:
This was a routine, unannounced inspection of a medical clinic located in Flint Michigan. The licensee was authorized to use byproduct material for diagnostic uptake, imaging, and localization studies. The nuclear medicine department was staffed with a full-time nuclear medicine technologist (NMT) who performed approximately 4-5 cardiac stress tests on Mondays, Tuesdays, and Thursdays. The NMT works from 7 am - 5 pm, Monday -Thursda The licensee retained the services of a medical physics consultant to conduct audits of the program on a quarterly basi PERFORMANCE OBSERVATIONS This inspection consisted of a tour of the nuclear medicine department; interviews with select licensee personnel; a review of select records; an observation of security of the materials; and independent measurements. The inspector observed an administration of a technetium-99m (Tc-99m) unit dose stress test. The inspector verified the location of sealed sources from the licensee's most recent inventory, and all sources were accounted for. The inspector had the NMT demonstrate the dose calibrator constancy check, package receipt procedures, the end of the day daily and weekly area surveys, proper handling of radioactive waste, and disposal procedures. Through these demonstrations and discussions, the inspector found that the licensee personnel were knowledgeable of radiation protection principles, licensee procedures, and regulatory requirement The inspector reviewed the following records: quarterly physicist audits, package receipts, waste disposal records, DOT Hazmat training, linearity and accuracy of the dose calibrator, instrument calibration, spill report, sealed source leak tests and inventory, daily area surveys, weekly wipe tests, and dosimetry records. The inspectors performed independent and confirmatory radiation measurements which indicated results consistent with licensee survey records and posting No violations of NRC requirements were identified as a result of this inspectio NRC FORM 592M (10-04-2022)
