901 Givens Road, Mount Vernon, IN 1 Scope and Observations:

Building Materials Manufacturing, d/b/a GAF Materials, was a roofing products manufacturer authorized to use sealed sources of byproduct material in fixed gauging devices to control elements of the asphalt shingle manufacturing processes at its facility in Mount Vernon, Indiana. At the time of the inspection, the licensee possessed two specifically-licensed devices containing Cs-137: a Thermo Process Instruments 5202 pipe-mounted gauge for asphalt density measurements, and a Thermo Fisher 5034 belt weigh scale gauge for measuring the quantity of shingle granules to be applied. The licensee also possessed two registered, generally-licensed beta gauges containing Sr-90 for final product thickness measurements. One, a Mahlo America QMS-12 (containing a model 11-200933 source holder) was commissioned in January 2025 to replace a Thermo EGS Gauging 21 Plus (containing a model SU-P77A source holder) which was out of service at the time of the inspection, awaiting decommissioning. The RSO performed shutter checks of the specifically-licensed devices quarterly and collected leak tests (analyzed by the manufacturer) semiannually. The RSO also served as responsible individual for the generally-licensed devices. Manufacturer field service personnel (working under reciprocity) performed shutter checks, leak tests, and other routine maintenance on these devices every six month The inspector toured the facility in Mount Vernon. All areas were adequately posted, and all licensed material was adequately secured to process equipment. All four devices were in good condition, adequately labeled, and installed in environments consistent with the conditions of normal use in the applicable SSDR safety evaluations. Independent surveys in the vicinity of each gauge were consistent with radiation profiles in their evaluations. No readings in excess of limits to members of the public were noted in unrestricted areas. The inspector discussed and reviewed the conduct of physical inventories, shutter checks, and sealed source leak tests with the RSO, who was knowledgeable of the radiation hazards present as well as the licensee's procedures for gauge use and tracking mechanisms for gauge maintenance. The inspector reviewed a selection of records, including inventory and shutter check PMs, leak test results, commissioning documentation for the Mahlo beta gauge, and the latest program audi No violations of NRC requirements were identified as a result of this inspectio NRC FORM 592M (10-04-2022)
