NRC 60-day Telephone Notification On 12/03/03 at 1438, Operations completed a 60 day telephone notification In lieu of an LER in accordance with guidance provided in 10CFR50.73(a)(1) for two separate events involving PCIS Group 3 isolations [CR-VTY-2003-02389 and 02471]. A §50.72 Notification was not required for these events as the system actuations occurred as the result of invalid signals.

DECEMBER 3,2003 NRC Telephone Notification Instructions The section below contains the Information necessary for Vermont Yankee (Docket 50-271) Operations to perform a 60-day telephone notification to the NRC Operations Center concerning Group III isolations that occurred on October 31, 2003 and November 9, 2003, with guidance provided in AP 0156. Ensure this notification has been endorsed by the Operations Superintendent and sign below with the date and time when complete.
1 0CFR50.73 (a)(1) provides for an optional 60-day telephone notification for certain invalid actuations, as an alternative to submitting a Licensee Event Report. This notification is being made pursuant to 10CFR50.73 (a)(2)(iv)(A).
A separate copy of this report has been prepared that provides the quoted Information below to fax to the NRC if requested.
NRC Telephone Notification Approved Statement
'This notification is being made in accordance with 10CFR50.73 (a)(2)(iv)(A), to provide the NRC with information pertaining to the Primary Containment Isolation System (PCIS) Group m invalid actuation signals-that affected containment isolation valves in more than one system on two separate occasions.
On October 31,.2003 and November 9, 2003, with the reactor at full power, an invalid actuation of the "B" Reactor Building Ventilation Monitor' caused Group m isolations of PCIS. A spike that was caused by electronic noise within the detector invoked a spurious High Level Trip to this monitor (RM 17-452B), that resulted in a trip to PCIS logic Channel B1.
- The PCIS functioned successfully providing a complete Group mlI isolation.
- The train actuation was complete. Both trains of the Standby Gas Treatment System started as designed. The PCIS Group M-isolation occurred which involves isolation of valves in the following systems:
o Drywell Air Purge and Vent o Drywell and Suppression Chamber Main Exhaust o Suppression Chamber Purge and Vent, o Containment Air Compressor Suction Valve, o Exhaust to Standby Gas Treatment, o Containment Purge Supply and Makeup, o Containment Air Sampling,
o Air dilution Subsystem Valves, o Vent Subsystem Valves, o Containment Air Dilution Vent System MOV VG-22 A/B It was determined that both of these Group El[ isolations were invalid, due the Reactor Building Vent Monitor Radiation Level Trip occurring at 25 mr/hr on 10/31/03 and 15 mr/hr on 11/09/03. General area radiation levels in these areas at the time of the vent were approximately 2.5 mr/hr to 6 mr/hr."
