Enclosed for your review is the
SALP report for the Palo Verde Nuclear
Generating Station. for the period December
1,
1994,
through June
1,
1996.
A
public meeting to discuss this report with you and your staff has
been
scheduled for 9 a.m.
(MST) on July 24,
1996. at the Energy Information Center
at the Palo Verde site.
Ouring this meeting you are encouraged
and expected
to candidly comment
on our report.
Although this meeting is
a forum between
Arizona Public Service
Company
and the
NRC, it will be open to observation
by
members of the public and other interested parties.
In accordance
with NRC policy, I have reviewed the recommendations
of the
Board and concur with their ratings
and views.
The operations,
maintenance,
and engineering
functional areas
have
been rated
as Category
1, reflecting
superior safety performance.
The plant support functional area
has
been rated
as Category 2, reflecting overall
good safety performance.

It is cle'ar that Aiizonf Public Seivice'has
established
the
programs
and processes
necessary
to achieve
and sustain
superior
performance.
Management attention is evident at All levels:
self-assessments
have
been
fully assimilated
throughout the Palo Verde organization:
and. self-criticism
has
been institutionalized
as
a path for individual and organizational
improvement.
However, additional challenges
remain to maintain
a consistent
and sustained
level of performance.
especially in the area of individual
attention-to-detail.
In the operations
area.
overall performance
improved sufficiently to achieve
a
Category
1 rating;
however.
the
SALP Board was not unanimous in reaching this
decision.
The considerable
actions
undertaken to improve operational
performance
and provide the operations
organization
leadership
necessary
to
achieve superior performance
throughout the site were recognized.
However,
improvements in procedure
use
and adherence.
and operator attention to details
are necessary
to address
inconsistencies
observed
in individual performance
and assure
continued superior performance.

In the maintenance
area.
safety performance
was also recognized
as having
improved to the superior levels
as demonstrated
by the improved material
condition of the primary plant and the low number of maintenance-induced
plant
challenges.
Management attention
and the skills displayed
by craft personnel
were significant contributors to the improvement in this area.
During the assessment
period.
the engineering
area
demonstrated
superior
safety performance.
The high quality of engineering
work products
was clearly
evident in the support provided in resolving problems with the site's
round
cell batteries
and the Unit 2 stuck fuel assembly.
Numerous engineering
programs'uch
as the steam generator
tube degradation
and thermography
programs,
were indicative of the superior contributions to safe plant
operations.
The plant support area continued to perform at
a good safety level.
The
emergency
preparedness
were seen
as areas of
strength:
however,
inconsistencies
in the implementation of the radiation
protection
and security programs
presented
continuing challenges
and detracted
from the overall functional
area rating.
Continual deficiencies
were
identified with the surveying
and posting of radiation and contamination
areas,
and ongoing difficulties were noted in the administration of the access
authorization
program.
In accordance
with 10 CFR 2.790 of the NRC's "Rules of Practice,"
a copy of
t;his letter and its enclosures will be placed in the
NRC Public Document
Room.
