This refers to the Nuclear Regulatory Commission

(NRC) inspection

conducted

by

D.

R. Carpenter

on March

16 - April 16,

1990.

The inspection

included

a review

of activities authorized for your Browns Ferry facility.

At the conclusion of

the inspection,

the findings were discussed

with those

members of your staff

identified in the enclosed

inspection report.

Areas

examined

during the inspection

are identified in the report.

Within

these

areas,

the inspection

consisted of selective

examinations

of procedures

and

representative

records,

interviews with personnel,

and observation

of

activities in progress.

The inspection

findings indicate certain activities

appeared

to violate

NRC

requirements.

The violation involved failure to take

compensatory

samples

required

by the

Technical

Specifications.

The violation, references

to

pertinent

requirements,

and

elements

to

be

included in your response

are

described

in the enclosed

Notice of Violation.

Consideration

was given to not issuing this violation since it was identified

by TVA and

was

a Level

V violation.

A review of records

revealed that you had

identified'hree

previous,

almost

identical,

events

in

LERs

259/88-41,

259/88-51

and 296/88-06.

The corrective actions

described

in these

LERs, if

implemented

properly,

should

have prevented this violation.

In your response

give particular attention to the root cause

determination

and the actions to be

taken to prevent recurrence of this problem.
