Dear Mr. Miltenberger:
SUBJECT: NRC INSPECTION REPORT NOS. 50-272/91-05; 50-311/91-05; 50-353/91-04
This letter transmits our findings of a routine inspection conducted by Mr. T. Johnson and
others of this office on February 13, 1991 through March 26, 1991. The inspection included
a review of activities authorized for the Salem and Hope Creek Generating Stations. At the
conclusion of the inspection, the findings were discussed with Messrs. S. LaBruna and J.
Hagan of you staff.
Areas examined during the inspection are identified in the report. While no violations are
identified for this period, several unresolved items were recognized as requiring further
review and evaluation by our staff, and will be addressed in subsequent inspections. We are
particularly interested in the circumstances in which a safety injection charging pump at
Salem was operated after maintenance, with the suction valve tagged closed. While not
affecting plant safety or operability at the time, the event appeared to be the result of multiple
personnel errors and procedural weaknesses in your operating and maintenance control
systems, to which management attention should be directed. Consequently, we will review
your evaluation of the adequacy of the planning and execution of the activities associated with
this event in the the next inspection period.
