Description of Event

On October 18, 1979, during refueling operations, containment integrity was violated when the inside and outside pipe capa were removed from'a containment penetration.

This event is contrary to T.S. 3.9.4 and reportable pursuant t o T. S. 6.9.1.9.d.

Probable Consequences of Occurrence The consequences of this event were tainimal because all operations liriolving core niterations or movement of irradiated fuel in the containment building were immediately suspended.

As a result, the health and safety of the general public were not affected.

There are no generic implications associated with this event.

Cause of Occurrence The design change which breached the containment penetration had been properly authorized.

Following this, core alterations began and con-struction personnel working this design change exited the containment.

A coordination problem developed between the individual who authorized the work and refueling personnel.

Immediate Corrective Acton All refueling operations were suspended and a plug was installed in the auxiliary building side of the penetration.

Scheduled Corrective Action No scheduled corrective action is required.

Actions Taken to Prevent Recurrence The proper steps in the refueling procedure will be changed to prevent breaking containment integrity because of penetration work during core alterations.
